FN及PTEN在肝细胞肝癌中的表达和临床相关性研究 被引量:4

Expression of FN and PTEN in hepatocellular carcinoma and its clinical significance

摘要 目的探讨抑癌基因纤黏连蛋白(FN)、张力蛋白同源基因蛋白(PTEN)在肝细胞性肝癌(HCC)组织中的表达及其临床意义。方法收集215例HCC、癌旁组织和19例正常肝组织,用qRT-PCR、免疫组织化学SP法检测FN、PTEN mRNA和蛋白表达,采用SPSS 13.0统计软件进行数据统计处理,FN及PTEN蛋白计量资料以x珋±s表示,采用t检验;阳性率及与HCC临床病理特征采用χ2检验。以P<0.05为差异有统计学意义。结果肝癌组织中FN mRNA和蛋白表达均明显高于癌旁组织及正常肝组织;而PTEN低于癌旁组织及正常肝组织(F=142.334,P<0.05),且在癌旁组织中的表达亦明显低于正常肝组织(F=80.861,P<0.05)。免疫组织化学检测,FN蛋白表达阳性率:HCC中为85.11%明显高于癌旁组织及正常肝组织(66.05%和10.53%),三者比较差异有统计学意义(χ2=58.671,P=0.000);PTEN蛋白表达阳性率:正常肝组织中(100%)明显高于HCC及癌旁组织(18.14%和46.97%),三者比较亦有统计学意义(χ2=75.271,P=0.000)。FN和PTEN与患者生存密切相关,术后12个月及24个月,FN表达阳性患者生存率明显低于阴性患者(89.77%与100%,83.52%与97.44%);而PTEN表达阳性患者的生存率高于阴性患者(98.39%与90.20%,96.77%与86.93%),两者之间差异均有统计学意义(P<0.05)。结论 FN及PTEN在肝癌组织中存在异常表达,FN表达提示患者预后不良,PTEN表达提示患者预后较好。 Objective To detect the expression of fibronectin (FN) and phosphatase and tensin homology deleted on chromosome ten (PTEN) in hepatocellular carcinoma (HCC)and to investigate its clinical significance. Methods Specimens were taken from 215 cases of tumors and adjacent tissues of HCC and 19 cases of normal liver tissue. The mRNA level and protein expression of FN and PTEN were detected using qRT-PCR and immunohistochemical SP method respectively. The correlation between expression of FN and FFEN and clinicopathologic features in HCC was analyzed. Statistical analysis was performed using the SPSS13.0 software. FN and FFEN proteins were expressed as mean ± SD and examined using Student' s t test, whereas the positive expression rate of FN and PTEN proteins and their relationship with clinical pathological features of HCC were examined using the Chi-square test. P 〈 0.05 was considered statistically significant Results The expression of FN at both mRNA and protein levels in tumor was significantly higher than tat in adjacent tissue or in normal liver tissue. However, the expression of PTEN at both mRNA and protein levels in tumor was lower than that in adjacent tissue or in normal liver tissue ( F = 142. 334, P 〈 0.05). The expression of PTEN in adjacent tissue was significantly lower than that in normal liver tissue ( F = 80. 86l, P 〈 0.05 ). When immunohistochemical staining was used, the positive expression rate of FN protein in HCC was 85. 11% , and it was significantly higher than that in adjacent tissue and normal liver tissue (66.05% and 10. 53% respectively ) (χ^2 = 58. 671, P = 0. 000 ). The positive expression rate of PEN protein in normal liver tissue ( 100% ) was significantly higher than that in HCC and adjacent tissues (18. 14% and 46. 97% ), (χ^2 = 75. 271, P = 0. 000). FN and PIEN were related to the postoperative survival of patients. The follow-up of 12 months and 24 months after surgery demonstrated that the survival rate of patients with positive FN expression was significantly lower than that of patients with negative FN expression (89.77% and 100%, 83.52% and 97.44% respectively). However, the survival rate of patients with positive PTEN expression was higher than that of patients with negative PTEN expression (98.39% and 90. 20%, 96. 77% and 86.93% respectively) (P 〈 0. 05). Conclusions FN and PTEN were abnormally expressed in HCC. The positive expression of FN predicts poor prognosis,but positive PTEN indicates fair prognosis.
