摘要
【目的】探讨磁共振IDEAL-IQ序列对肝脏脂肪变性和铁过载的诊断价值。【方法】对20例脂肪肝患者(脂肪肝组),20例地中海贫血反复输血患者(贫血组),以及20例健康体检者(对照组)行3.0T MRI的IDEAL-IQ序列扫描,通过脂肪比像、R2*弛豫率像对肝脏的脂肪含量、铁含量进行定量测定。【结果】脂肪肝组的肝脏脂肪含量为(15.28±5.75)%,对照组的脂肪含量为(3.45±1.81)%,两者之间差异有统计学意义。贫血组的肝脏R2*值为(592±115)Hz,对照组R2*值为(37±15)Hz,两者之间差异亦有统计学意义。此外,有2例贫血组患者脂肪含量测定分别为15%、45%。【结论】磁共振IDEAL-IQ序列一次扫描即获得多组图像,其中脂肪比像、R2*弛豫率像可以对肝脏内的脂肪含量、铁含量行定量分析,对肝脏铁过载患者的肝脂肪变性诊断具有重要临床价值。
【Objective】 The purpose of this study was to discuss the diagnosis value of IDEAL-IQ sequence to hepatic steatosis and hepatic iron overload in the patients. 【Methods】 20 patients with fatty liver(fatty liver group), 20 cases of thalassemia patients with repeated blood transfusion(anemia group), 20 healthy subjects(control group) were obtained using IDEAL-IQ sequence on a 3.0T scanner. Fat ratio image, R2 * relaxation rate image were used to diagnosis whether there is hepatic steatosis or hepatic iron overload. 【Results】 The fat content of fatty liver group and control group were 15.28 ±5.75% and 3.45 ±1.81%, respectively.Statistically significant difference was obtained. The R2* value of anemia group and control group were 592.15±115.31 Hz and 36.56±14.6Hz, respectively. There was obviously statistical difference between them. In addition, the fat content of 2 patients in anemia group were 15% and 45% respectively. 【Conclusion】 There were several group images once IDEAL-IQ sequence was scanned, fat ratio image, R2* relaxation rate image could be used to quantitative diagnosis hepatic steatosis and hepatic iron-overloaded, there was important clinical value for the diagnosis of hepatic steatosis in the hepatic iron-overloaded patients.
