桂枝茯苓胶囊联合米非司酮治疗子宫腺肌病临床研究

Effect of Guizhi Fuling capsules in combined with mifepristone on treatment of adenomyosis

目的:对米非司酮联合桂枝茯苓胶囊治疗子宫腺肌病(AM)临床疗效及与性激素、糖类抗原125(CA125)、血管内皮生长因子(VEGF)相关性进行研究。方法:对照组于月经来潮第一天开始口服米非司酮12.5mg/d,一日一次,观察组在对照组基础上口服桂枝茯苓胶囊3粒/次,一日三次,疗程为6个月,疗程结束后对比观察两组临床效果和子宫体积、痛经情况、月经量变化情况,以及血清性激素(E2、FSH、LH、P)、血清CA125、VEGF水平的变化。结果:观察组总有效率明显高于对照组,差异有统计学意义(P<0.05);治疗组E2、FSH、LH、P水平下降程度优于对照组,差异有统计学意义(P<0.05);观察组子宫体积、痛经程度、月经量改善程度优于对照组,差异有统计学意义(P<0.05);观察组血清CA125、VEGF水平下降程度优于对照组,差异有统计学意义(P<0.05)。结论:米非司酮联合桂枝茯苓胶囊治疗AM能有效改善患者性激素水平,缩小子宫体积,改善痛经程度及减少月经量,临床检测血清CA125和VEGF水平,对AM的诊断和病情转归有重要意义。

Objective：To explore the clinical efficacy of mifepristone in combined with Guizhi Fuling capsules in the treatment of adenomyosis（AM）and their correlations with hormone,CA125,and VEGF.Methods：The patients in the control group were orally given mifepristone（12.5mg/d）the first day of menstrual cycle,once a day.The patients in the observation group were given mifepristone in combined with Guizhi Fuling capsules,3particles/time,three times a day.Six-month was taken as one course.After the course,the clinical effect,uterine volume,dysmenorrhea,menstrual blood volume,and the levels of hormones（E2,FSH,LH,and P）,CA125,and VEGF in the two groups were observed and compared.Results：The total effective rate in the observation group（93.33%）was significantly higher than that in the control group（78.33%）（P〈0.05）.The decreased degree of E2,FSH,LH,and P levels in the observation group was significantly superior to that in the control group（P〈0.05）.The improvement degree of uterine volume,dysmenorrhea,and menstrual blood volume in the observation group was significantly superior to that in the control group（P〈0.05）.The declining degree of CA125,and VEGF levels in the observation group was significantly superior to that in the control group（P〈0.05）.Conclusions：Mifepristone in combined with Guizhi Fuling capsules in the treatment of AM can effectively enhance the hormone level,shrink the uterine volume,improve the dysmenorrheal degree,and reduce the menstrual blood volume.Clinical detection of the levels of serum CA125,and VEGF is of great significance for the diagnosis and outcome of AM.
