膝骨关节炎软骨下骨微灌注、血管侵入及软骨退变相关研究 被引量:4

摘要 目的:研究膝骨关节炎患者胫骨平台、股骨远端软骨下骨微灌注改变与软骨下骨血管侵入、软骨损伤相关性。方法:选取全膝关节置换术患者20例,术前接受动态增强MRI检查,测得内外侧胫骨平台、内外侧股骨远端4个相近规格的软骨下骨兴趣区域的强化率(ER),最大斜率(MU)。术后按兴趣区域在胫骨内外侧平台中心、股骨内外髁远端取2 cm×2 cm骨块,脱钙后HE染色,关节软骨受损程度采用Mankin评分。软骨下骨血管侵入数目采用高倍镜随机视野平均血管计数法。比较兴趣区胫骨、股骨软骨下骨微灌注差异、血管侵入数、Mankin评分差异,以及各指标的相关性。结果:Mankin评分胫骨侧大于与股骨侧(P<0.05),提示胫骨侧软骨退变较股骨侧严重。ER、MU内侧间室大于外侧间室(P<0.05),提示内侧间室软骨下骨微灌注大于外侧间室。内侧股骨远端ER与内侧股骨HE血管数,内侧股骨远端MU与内侧股骨HE血管数,内侧胫骨平台ER与内侧胫骨HE血管数呈正相关。结论:内翻畸形的膝骨关节炎患者,内侧间室的ER、MU较外侧显著增大,提示内侧软骨下骨呈高灌注状态,并通过相关性比较表明,高灌注状态与软骨下骨血管侵入数目相关。
