摘要
目的探究颌骨骨折应用微型钛板坚固内固定联合颌间牵引钉颌间牵引治疗的临床有效性,分析其安全性。方法将2012年2月至2014年12月期间,因颌骨骨折入院接受治疗的82例患者纳入本次研究,随机均衡分为研究组和对照组,各41例,研究组采用颌间牵引钉进行颌间牵引同时结合微型钛板坚固内固定进行治疗,对照组采用传统牙弓夹板行颌间结扎术,对比两组患者临床治疗有效性及安全性。结果研究组显效26例,有效15例,无效病例0例,治疗总有效率100%,对照组显效16例,有效14例,无效11例,总有效率为73.17%,组间差异有统计学意义(x^2=13.415,P=0.001<0.05);两组患者在接受治疗后菌斑指数(PI)、软垢指数(DI)、牙龈指数(GI)均出现一定程度提高,差异均有一定统计学意义(P<0.05),对照组升高程度更为明显(P<0.05);对比两组患者治疗后不良事件发生情,对照组术后不良事件总发生率(41.46%)高于研究组的(4.88%),其中在牙齿损伤、感染、骨折移位等数据差异有显著统计学意义(P<0.05),排异反应无显著统计学意义(x^2=2.050,P=0.152<0.05)。结论颌间牵引钉牵引联合微型钛板坚固内固定术治疗颌骨骨折的效果较好,能使下颌骨临床功能和口腔功能较好地恢复,降低不良反应,提高安全性。
Objective To explore the effect and safety of internal fixation with mini titanium board strong nail joint between jaw traction in treatment of jaw bone fracture. Methods 82 patients with jaw fracture during February 2012 to December 2014 was included in this study, They were divided into research group and the control group, each 41 cases. The patients in research group were treated with jaw traction nail between traction in combination with mini titanium board strong internal fixation, and in control group were treated with traditional dental arch splint ligation between lines of the jaw. Cinical efficacy and safety were compared between the two groups. Results In research group, 26 cases were markedly improved, 15 cases effective, and the total effective rate was 100%. In control group, 16 cases were marked effect, 14 cases effective, 11 cases ineffective, the total effective rate was 73.17%, the difference between two groups was statistically significant (χ2=13.415, P=0.001〈0.05). After treatment, the plaque index (PI), soft dirt index (DI), gingival index (GI) periodontal indexes were appeared a certain degree, the differences had statistical significance ( P〈0.05), the degree of control group increased more obviously (P〈0.05). Compared with adverse events after treatment between two groups, total incidence of postoperative adverse events in control group (41.46%) was higher than that in research group (4.88%), including in tooth injury, infection, fracture displacement,data differences was significant ( P〈0. 05). There was no significant statistical significance in rejection (χ2=2.050, P=0.152〈0.05). Conclusion The effect of nail traction between the jaw joint mini titanium board strong fixation for the treatment of jaw fracture was good, it could make the mandibular clinical function and oral function better recovery, reduce adverse reactions, and improve security.
