四蚕加味方辨证治疗IgA肾病的临床观察

Clinical Observation of Si Can Jia Wei Formula Compound in Treating IgA Nephropathy

目的:观察四蚕加味方治疗Ig A肾病的临床疗效。方法:将61例气阴两虚型Ig A肾病(CKD1-3期)患者随机分为2组,治疗组31例采用四蚕加味方联合固本通络方治疗,对照组30例采用固本通络方治疗,疗程2个月。结果:治疗组临床总疗效优于对照组(P<0.05);治疗组24 h尿蛋白定量、补体C3、T细胞(CD4、CD8、CD4/CD8)的改善显著优于对照组(P<0.01),尿沉渣红细胞、免疫球蛋白(Ig G、Ig A)的改善优于对照组(P<0.05)。结论:四蚕加味方能够显著改善气阴两虚型Ig A肾病患者的临床症状,减少血尿和蛋白尿,改善体液免疫和细胞免疫功能,体现了"从风论治"慢性肾炎的应用价值。

Objective： The aim of this study was to investigate the effect of Periostracum Cicadae compound in Ig A nephropathy patients with Qi and Yin deficiency syndrome in chronic kidney disease（ CKD） stage 1- 3. Methods In this randomized controlled clinical study,61 patients were enrolled. They were randomized into treatment group（ 31 cases,Periostracum Cicadae compound with Guben Tongluo Decoction） and control group（ 30 cases,Guben Tongluo Decoction）,with a two- month course of therapies.Results： The entire effects were improved significantly in treatment group than those in control group（ P〈0. 05）. When compared the results of control group,proteinuria,C3,CD4,CD8,and CD4/CD8 were more significantly improved（ P〈0. 01）. Hematuria,Ig G,and Ig A were also significantly improved in treatment group than those in control group during the study period（ P〈0. 05）.Conclusion： Periostracum Cicadae compound is effective to improve clinical symptoms,reduce hematuria and proteinuria and enhance humoral and cellular immunity in Ig A nephropathy patients with Qi and Yin deficiency syndrome. It reflects the applied value in wind- oriented treatment of primary chronic glomerulonephritis.
