摘要 目的:研究右美托咪定对麻醉诱导前QTc间期的影响。方法:择期行全身麻醉手术患者20例,年龄28~70岁,ASA分级Ⅰ-Ⅱ级,按照随机数字表法将患者均分为右美托咪定组(DEX组)和对照组(CON组),DEX组在麻醉诱导前10min内泵入1μg/kg DEX,CON组以同样方式泵入同速度同容量生理盐水。观察并记录两组患者用药前以及用药后第1、2、3、4、5、6、7、8、9、10 min的ECG、HR和MAP的变化,分别标记为T0、T1、T2、T3、T4、T5、T6、T7、T8、T9、T10。结果:两组患者QTc值DEX组内与CON组间比较,差异无统计学意义(P〉0.05),DEX组内比较T4~T10明显缩短,差异有统计学意义(P〈0.05);两组患者HR值DEX组与CON组间比较T6~T10明显下降,差异有统计学意义(P〈0.05),DEX组内比较T6~T10明显下降,差异有统计学意义(P〈0.05);MAP组间与组内比较,差异无统计学意义(P〉0.05)。结论:右美托咪定可以有效抑制麻醉诱导前期QT间期延长。 Objective: To investigate the effects of dexmedetomidine on QTc interval before anaesthesia induction. Methods: 20 cases of patients scheduled to undergo general anaesthesia,28 to 70 years old,ASA I-II were randomly divided into the observation group( DEX group) and control group( CON group) according to the random number table method. Patients in DEX group were allocated to receive infusion of dexmedetomidine by 1 μg / kg within 10 minutes before anaesthesia induction. In CON group,patients got infusion of saline in the same way. ECG,HR,MAP were recorded before drug injection and at the 1,2,3,4,5,6,7,8,9,10 min after injection,marked T0,T1,T2,T3,T4,T5,T6,T7,T8,T9,T10 respectively. Results: There was no statistical difference of QTc in two groups( P〉0. 05). In DEX group,QTc were significantly shorter at T4 to T10( P〈0. 05); Compared with CON group,HR in DEX group were significantly lower at T6 to T10( P〈0. 05),and also significantly lower at T6 to T10 within DEX group( P〈0. 05); There was no statistical significant difference in two groups of MAP( P〉0. 05). Conclusion: Dexmedetomidine may inhibit the prolongation of QTc interval effectively before anesthesia induction.
