超声检测三尖瓣环位移评价比较不同透析方式对右心室整体收缩功能的改善效果 被引量:1

摘要 目的应用超声检测三尖瓣环位移,探讨连续性血液透析(CHD)与间歇性血液透析(IHD)改善急性肾功能衰竭(ARF)右心室整体收缩功能的价值。方法收集87例急性肾功能衰竭接受维持性血液净化治疗的患者为观察对象,按照治疗方法的不同分为两组:间歇性血液透析组、连续性血液透析组。治疗前后,两组患者均检测尿素氮(BUN)、血清肌酐(SCr)及CO_2-CP、K、Na、Cl、Ca等血生化指标,应用斑点追踪技术(STI)观察三尖瓣环位移(TAPSE)。检测结果标准化后进行两两比较。结果治疗后两组患者的BUN、Scr下降幅度有明显差异(P<0.05),CO_2-CP、K、Na、Cl、Ca的测值均在正常范围,且无明显差异(P>0.05),TAPSE幅度也有明显差异(P<0.05),IHD组低于CHD组。结论 CHD及IHD治疗急性肾功能衰竭的过程中,CHD改善右心室收缩功能的作用明显优于IHD。 Objective To evaluate and compare the effect of improving the overall shrinkage function of the right ventricle in the process of treating acute renal failure (ARF) by continuous hemodialysis (CHD) and intermittent hemodialysis (IHD). Methods The 87 patients with acute renal failure treated maintaining blood purification therapy was collected and divided into two groups according to different treatment:the group of intermittent hemodialys and the group of continuous hemodialysis. Before and after treatment,in the two groups, the indicators of urea nitrogen (BUN),serum creatinine (SCr) and CO2-CP,K,Na,C1,Ca was tested,and observed displacement of tricuspid annulus(TAPSE) with spot tracking technology(STI). Test results of being standardized of the two groups was compared. Results After treatment,the dropped range of BUN,Scr had obvious difference (P〈0.05),the measured data of K,Na,C1,Ca have no difference (P〉0.05) in the two groups. And TAPSE of measured by STI of the two groups also have obvious difference (P〈0.05),IHD group is lower than the CHD group. Conclusion The effect of improving the overall shrinkage function of the right ventricle of CHD is better than that of IHD in treating acute renal failure with CHD or IHD.
