脑卒中患者吞咽功能评估与摄食护理

An Evaluation of Swallowing Function of Cerebral Infarction Patients and Ingestion Nursing

目的：探讨应用Gugging吞咽功能评估表（Gugging Swallowing Screen，GUSS）对脑卒中患者进行吞咽功能评估并给予吞咽干预及康复护理的效果。方法：运用GUSS评估脑梗死患者，对吞咽障碍患者进行吞咽干预和康复护理。结果：59例中存在吞咽障碍者36例，占61％。经过康复治疗及护理干预，出院前GUSS评估，20分者48例，15～19分者9例，10～14分者2例，无肺炎、营养不良等并发症导致的死亡病例。结论：床边GUSS评估简单、实用、可操作性强，医护人员依据患者吞咽障碍程度进行个性化、循序渐进的康复治疗及护理干预，降低误吸、营养不良等风险发生，对预后起到积极作用。

Objective:To study Gugging Swallowing Screen and effects of ingestion nursing on cerebral infarction patients Methods:Gugging Swallowing Screen had been applied to evaluate the cerebral infarction patients.Swallowing training had been provided for the patients with the swallowing disorder as well as oral cavity care,ingestion nursing,psychological care and comprehensive rehabilitation training.Results:Among 59 cases with the swallowing disorder,11 cases still suffered from the disorder after swallowing intervention and rehabilitation training when discharging from hospital.28 cases were in good condition by prognosis grading with Glasgow Outcome Scale,19 cases mildly disabled and 12 cases severely disabled.There were no ADLV cases or death cases by malnutrition.Conclusion:The swallowing disorder can be discovered as soon as possible by evaluating the patients through Gugging Swallowing Screen.Swallowing intervention and ingestion nursing can be provided immediately,which can effectively improve the prognosis and living quality of cerebral infarction patients.
