喉癌患者机体能量消耗及希望水平与生活质量关系研究

Organism energy consumption and relationship between hope level and life quality in patients with laryngeal cancer

目的对喉癌术后患者的希望水平和生活质量以及两者之间的关系进行研究和分析。方法以2010年2月至2013年10月收治的136例喉癌术后患者作为研究对象,分别采用Herth希望量表、欧洲癌症研究与治疗组织核心生活质量问卷(EORTC QLQ-C30)对患者的希望水平和生活质量进行评价,对两者之间的相关性进行Pearson分析,并对患者机体能量消耗变化进行观察和分析。结果术后患者在整体健康状况、功能、症状以及各单项评分上均较术前明显提高(P<0.05或P<0.01),其中65.5%的患者表现为低等希望水平,31.6%的患者为中等希望水平,仅有2.9%的患者表现为高等希望水平。而Pearson相关性分析显示希望水平各维度与患者整体健康状况、功能、症状以及单项均呈正相关(P<0.05,P<0.01)。结论喉癌患者术后希望水平越高,其术后生活质量则越高,因此临床可加强对患者术后希望水平的关注,制定相应措施,以提高患者的生活质量。

Objective To study the relationship between hope level and life quality and observe the changes of organism energy consumption in postoperative patients with laryngeal cancer. Methods Total 136 postoperative patients with laryn- geal cancer who were treated from February 2010 to October 2013 were selected as the research objects. Herth hope index was applied to evaluate the hope level of patients, and the questionnaire of life quality of the organization of European canc- er research and treatment（ EORTC QLQ-C30）was used to assess the life quality of the patients before and after surgery. The correlation of the hope level with life quality was estimated by Pearson analysis. Food frequency of patients were collected by food frequency questionnaire （FFQ）. The changes of total energy consumption and fat density were observed analyzed by FFQ. Results The scores of EORTC QLQ-C30 for overall health status, body function, clinical symptoms and each single item score of patients after surgery were significantly higher than those before surgery （ All P 〈 0.05 ）. Herth hope index showed that 65.4% patients were in lower hope level;31.6% patients in medium hope level;only 2.9% patients in higher hope level. The Pearson correlation analysis showed that each factor of the hope level was positively related with overall health status, body function, clinical symptoms and single item （ All P 〈 0.05 ）. FFQ showed that diet fat density had certain influence on short term energy consumption. Conclusion The higher of hope level of postoperative patients with laryngeal cancer, the better of postoperative life quality. So more concerns should be put on the inereasing of the level of hope to im- prove the quality of patients＇ life.
