经胸超声心动图与经主动脉造影指导动脉导管未闭封堵治疗的对比研究

A comparative study on transcatheter closure of patent ductus arteriosus guided by transthoracic echocardiography and angiography

目的对比分析经胸超声心动图(TTE)与经主动脉造影(TAA)指导动脉导管未闭(PDA)封堵治疗的效果。方法选择欲行PDA介入封堵治疗的患者82例,随机分为TTE组52例和TAA组30例。全部患者术前均用超声心动图准确测量PDA内径。比较两组患者的封堵成功率及相关情况。结果超声测量PDA内径与所选择封堵器大小相关性良好(r=0.895,P<0.01)。TTE组和TAA组的PDA内径分别为(4.0±1.4)mm和(3.6±1.5)mm,手术成功率分别为96.15%和100%,TTE组手术操作时间、平均住院时间、造影剂用量、X线曝光量及住院总费用均小于TAA组,差异均有统计学意义(P<0.05)。结论 TTE指导PDA封堵治疗是一种安全可行的方法,值得临床推广应用。

Objective To compare the efficiency of transthoracic echocardiography（Tl＇E） and angiography（TAA） in guiding transcatheter closure of patent ductus arteriosus（PDA）.nethods Eighty-two patients with PDA who would undergo percutaneous transcatheter closure were divided into two groups：the 33＂E group（52 patients）and the TAA group （30 patients ）.The PDA diameter of all the patients were measured by echocardiography before operation.The procedure successful rate and other related conditions were compared between the two groups.Results A significant correlation existed between the PDA diameter measured by TIE and the size of the used occluder（r=0.895,P〈0.001）.The PDA diameter of the TIE group and the TAA group was （4.0±1.4） mm and （3.6＋1.5） ram,the successful rate was 96.15% and 100%.But the time of closure procedures,the average time of hospital stays,the injection volume of contrast medium,the quantity of X-ray exposure and the expenses was significantly smaller in TrE group than those in TAA group （P〈0.05）.Conclusion Compared with guidance by TAA,transcatheter closure of PDA under the guidance of TIE is more efficient and safe.
