持续气道正压通气治疗对重度阻塞性睡眠呼吸暂停低通气综合征患者血红蛋白及血清高敏C反应蛋白水平的影响

Effect of continuous positive airway pressure on levels of hemoglobin and high sensitivity C-reactive protein in patients with severe obstructive sleep apnea-hypopnea syndrome

目的 探讨持续气道正压通气（CPAP）治疗对重度阻塞性睡眠呼吸暂停低通气综合征（OSAHS）患者血红蛋白及血清高敏C反应蛋白（hs-CRP）水平的影响.方法 选择2012年7月至2014年2月河北省唐山市丰南区医院重度OSAHS患者30例为观察组,27例健康体检志愿者为对照组,详细询问病史,对所有研究对象于晨起采集静脉血标本,测定血红蛋白及hs-CRP水平,比较观察组与对照组间血红蛋白、hs-CRP水平的差异;观察组进行CPAP治疗并随访,6个月后复查上述各项指标,比较观察组经CPAP治疗后血红蛋白及hs-CRP水平的变化.结果 治疗前观察组血红蛋白及hs-CRP水平明显高于对照组,差异有统计学意义[（146 ±7） g/L比（132 ±8） g/L、（5.0±1.7） mg/L比（2.5±1.4） mg/L,P＜0.05];观察组进行CPAP治疗后6个月随访测定血红蛋白、hs-CRP水平分别为（132±9）g/L、（3.8 ±1.1）mg/L,较治疗前明显降低,差异有统计学意义（P＜0.05）.结论 CPAP治疗可降低重度OSAHS患者血红蛋白、hs-CRP水平,血红蛋白、hs-CRP可作为OSAHS患者炎性反应严重程度及疗效判断的参考指标。

Objective To investigate the effect of continuous positive airway pressure （CPAP） on levels of hemoglobin and high sensitivity C-reactive protein （hs-CRP） in patients with severe obstructive sleep apneahypopnea syndrome （OSAHS）.Methods Thirty severe OSAHS patients （observation group） and 27 healthy volunteer （control group） from July 2012 to February 2014 were enrolled.The levels of hemoglobin and hs-CRP were compared between groups.After.6 months of treatment, the levels of hemoglobin and hsoCRP in observe group were reexamined and compared with those before treatment.Results The levels of hemoglobin and hs-CRP in observe group were significantly higher than those in control group before treatment [（146 ± 7） g/L vs （132 ± 8） g/L, （5.0 ± 1.7） mg/L vs （2.5 ± 1.4） mg/L] （P 〈 0.05）;the levels of hemoglobin and hs-CRP in observe group were significantly reduced after 6 months of treatment [（132 ± 9） g/L, （3.8 ± 1.1） mg/L] （P 〈 0.05）.Conclusions CPAP can significantly reduce the levels of hemoglobin and hs-CRP in severe OSAHS patients;hemoglobin and hs-CRP can be used as indexes to judge the severity of inflammation and curative effect in severe OSAHS patients.
