医院2012-2014年静脉药物配置中心不合理用药回顾性分析

Analysis of irrational drug use from 2012 to 2014

目的 对河北北方学院附属第一医院2012年1月至2014年12月静脉药物配置中心不合理用药医嘱进行分析汇总,以便临床查阅.方法 对我院静脉药物配置中心登记在册不合理用药的数据,进行整理、统计并分析.结果 2012-2014年不合理用药医嘱占全年总调配用药医嘱比例逐年下降,分别为0.082％ （287/348 880）、0.052％（192/366 267）、0.026％（109/415 862）.不合理用药的主要类型为：溶媒选择不当、配伍不当、用法用量不当、用药衔接不合理、电脑录入错误等.结论 药师在审读药方时发现不合理用药医嘱及时进行有效干预,可以使患者的用药安全得到保障。

Objective To analyze the irrational drug use from January 2012 to December 2014 in the first affiliated hospital of Hebei north university hospital.Method Data of irrational drug use which registered in intravenous drug distribution center were retrospectively analyzed.Result Irrational drug use declined yearly 2012-2014, accounting for 0.082% （287/348 880）, 0.052% （192/366 267） and 0.026% （109/415 862）,respectively.The main types of irrational use included improper solvent, inappropriate compatibility, improper dosage, reasonable medication convergence and input errors.Conclusion Timely and effective intervention from the pharmacists for irrational drug prescription is a guarantee of safe medication.
