冠心病合并糖尿病患者载脂蛋白B-48水平与致病因素之间相关性研究

摘要 目的探讨冠心病合并糖尿病患者载脂蛋白B-48(Apo B-48)水平与各致病因素之间的相关性。方法选择冠心病患者20例作为冠心病组、糖尿病患者20例作为糖尿病组、冠心病合并糖尿病患者20例作为合并组,正常人20例作为正常组。检测4组血Apo B-48、体质量指数(BMI)、血压、空腹血糖、空腹胰岛素水平及血脂6项[总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(Apo A1)、载脂蛋白B(Apo B)],分析Apo B-48与其他因素之间的相关性。结果合并组Apo B-48水平明显高于其余3组(P均<0.05),冠心病组和糖尿病组均明显高于正常组(P均<0.05),冠心病组和糖尿病组间比较差异无统计学意义。正常组BMI低于其他3组(P均<0.05),其余3组间比较差异无统计学意义。合并组血压明显高于其他3组(P均<0.05),其余3组间比较差异无统计学意义。正常组、冠心病组、糖尿病组、合并组空腹血糖、空腹胰岛素且呈递增关系,各组间比较差异均有统计学意义(P均<0.05)。4组TC水平比较差异无统计学意义;合并组TG和LDL-C水平明显高于其他3组(P均<0.05);正常组Apo A1、Apo B水平明显高于其他3组(P均<0.05),其余3组间比较差异无统计学意义;糖尿病组HDL-C水平明显高于冠心病组和合并组(P均<0.05),余3组间比较差异无统计学意义。Apo B-48与BMI、收缩压、TC、TG以及Apo B呈正相关性(P均<0.05)。结论血Apo B-48水平与代谢性疾病,特别是脂类代谢性疾病密切相关,检测该指标水平对该类疾病的诊治具有重要意义。 Objective It is to explore the correlation between the level of ApoB -48 and pathogenic factors in the patients with coronary heart disease (CHD) complicating with diabetic. Methods 80 cases of subjects were divided into four groups, CHD group, diabetes mellitus group, diabetic CHD group and normal group, each had 20 eases. BMI, blood pressure, ApoB - 48, blood lipids including TC, TG, HDL- C, LDL- C, ApoA1 and ApoB, fasting glucose, and fasting insulin levels were detected to explore the differences between the four groups as well as the correlation between ApoB - 48 and other factors. Results The highest levels of ApoB - 48 was found in merger group, and the normal group was the lowest ( P 〈 0.05 ) , but there was no significant difference between the coronary heart disease group and diabetes group. BMI in normal group was lower than the other three groups ( P 〈 0.05 ). The difference of blood sugar in four groups was increasing, the normal group was the lowest, CHD group was lower than the diabetes mellitus group, and the merged group had the highest (P 〈 O. 05 ). Blood pressure of combined group was significantly higher than other three groups (P 〈 0.05). The differences of fasting blood glucose, fasting insulin blood sugar in four groups was increasing, the normal group was the lowest, CHD group was lower than the dia- betes mellitus group, and the merged group had the highest ( P 〈 0. 05 ). The level of TC of four groups didnl have significant difference, but the levels of TG and LDL - C in complicating group were higher than that in the other three groups ( P all 〈 0. 05 ). The levels of ApoA1 and ApoB in normal group were higher than that in the other three groups in which the differences were not no significant. The level of HDL - C in diabetes mellitus group was higher than that in CHD group and complicating group (P all 〈 0. 05) , and the difference was not significant in the other three groups. Negative correlations were found be-tween ApoB -48 and BMI, systolic blood pressure, TC, TG and ApoB. Conclusion The level of blood ApoB -48 is closely related with metabolic diseases, especially with lipid metabolic disease, determination of this index has important significance for the diagnosis and treatment of the diseases.
