沙丁胺醇和利托君对先兆早产孕妇心血管系统及胎心率、胎儿脐动脉血流水平的短期影响分析 被引量:1

摘要 目的:探讨沙丁胺醇和利托君应用于先兆早产时,对孕妇心血管系统、胎儿心率、胎儿脐动脉血流水平的影响.方法:将120例先兆早产的孕妇随机分为沙丁胺醇组和利托君组,沙丁胺醇组52例用沙丁胺醇口服治疗,利托君组52例用利托君静脉给药治疗,比较两组孕妇心率、血压、胎心率及胎儿脐动脉血流水平情况.结果:两组孕妇的脉压差、心率在给药后均明显增高,并具有统计学意义,利托君组的增高更为显著,与沙丁胺醇组相比,其差异具有统计学意义.利托君组胎心率在用药后增加明显,其差异具有统计学意义,两组孕妇平均动脉压及胎儿脐动脉血流水平均无明显变化.结论:静滴利托君治疗明显使孕妇心率、脉压差及胎心率增加,使用时宜严密观察心率、血压及胎心率情况.口服沙丁胺醇副反应较轻微,对于不耐受利托君心血管副反应的患者可选用. Objective: The study investigated the effect of salbutamol and ritodrine on cardiovascular system of patients with threatened abortion, and on the fetal heart rate and the umbilical artery blood flow. Methods: 104 pregnant women with threatened premature labor were randomly divided into a salbutamol group and a ritodrine group. The 52 patients in the salbutamol group were treated with oral salbutamol and the 52 patients in the ritodrine group were treated with intravenous injection of ritodrine. The maternal heart rate, blood pressure, fetal heart rate, and fetal umbilical artery blood flow were compared between the two groups. Results: After administration, maternal pulse pressure difference and maternal heart rate in both groups increased significantly and the differences before and after the administration had statistical significance. The augmentation was significantly higher in the ritodrine group. Fetal heart rate rose significantly in the ritodrine group. Both groups had no significant changes in mean arterial blood pressure or fetal umbilical artery blood flow. Conclusion: Intravenous injection of ritodrine can significantly increase maternal pulse pressure, maternal heart rate, and fetal heart rate thus close monitoring of the maternal pulse pressure, maternal heart rate, and fetal heart rate shal be applied to prevent adverse reaction. Side reaction of oral salbutamol is mild and patients intolerant to cardiovascular side reactions caused by ritodrine may use oral salbutamol.
