超声造影增强模式与时间—强度曲线结合在甲状腺实性结节诊断中的价值 被引量:1

摘要 目的评估超声造影增强模式评分与时间—强度曲线评分在甲状腺实性结节良恶性鉴别诊断中的价值。方法甲状腺实性结节性疾病患者62例,共96个结节(所有结节性质均经手术病理证实)行超声造影检查,分析造影增强模式评分与时间—强度曲线评分对甲状腺实性结节的鉴别诊断价值。结果经受试者工作特征曲线分析,向心性增强模式评分曲线下面积为0.763,当向心性增强模式评分≥2分时,诊断甲状腺恶性结节(乳头状癌)的敏感性为83.3%,特异性为66.7%;时间—强度曲线评分曲线下面积为0.755,当评分≥4分时,诊断甲状腺恶性结节(乳头状癌)的敏感性为50.0%,特异性为91.9%。结论超声造影向心性增强模式评分对甲状腺实性结节良恶性的鉴别诊断价值大于时间—强度曲线评分。 Objective To assess the value of the contrast-enhanced ultrasound score and time-intensity curve score in diagnosis of benign and malignant thyroid nodules. Methods 62 patients with 96 thyroid solid nodules were enrolled into this study and checked by ultrasound(the final diagnosis was confirmed by pathologist after thyroidectomy). The value of contrast enhancement pattern grade, intensity-time curve parameters in differential diagnosis of thyroid benign and malignant nodules was analyzed. Results The ROC(receiver operating characteristic) curve analysis for contrast enhancement pattern grade indicated that area under the curve(maximum) by centripetal enhancement pattern grade is 0.763, when the centripetal enhancement pattern score≥ 2 points, the diagnosis sensitivity and specificity of malignant thyroid nodules(papillary carcinoma) is83.3% and 66.7% respectively. The area under the curve(maximum) by time-intensity curve are 0.755, when the score ≥ 4, the diagnosis sensitivity and specificity of malignant thyroid nodules(papillary carcinoma) are50.0%, 91.9% respectively. Conclusion The contrast- enhanced ultrasound centripetal enhancement pattern score is better than the time-intensity curve score in the diagnosis of benign and malignant thyroid nodules.
