肾嗜酸细胞腺瘤多层螺旋CT表现

The multi-slice spiral CT manifestations of renal oncocytoma: report of two cases and literature review

目的探讨肾嗜酸细胞腺瘤的多层螺旋CT表现，进一步提高对肾嗜酸细胞腺瘤诊断及鉴别诊断水平。方法结合相关文献，回顾性分析经病理证实2例肾嗜酸细胞腺瘤的多层螺旋CT表现。结果本组病例均位于肾下部．直径约3．8～5．2cm，CT平扫呈等、高密度，其中1例病灶内有星状瘢痕；增强扫描皮质期病灶明显强化，实质期及。肾盂期病灶强化程度减低，密度均匀，其星状瘢痕延迟强化。2例CT三维成像示肿瘤血管来源于肾下前动脉．其中1例可见动静脉短路。结论肾嗜酸细胞腺瘤多层螺旋CT具有一定的特征性，认识这些特点有助于提高诊断准确率；同时多层螺旋CT三维血管成像为临床制定术前方案提供重要依据。

Objective To explore the multi-slice spiral CT manifestations of renal oncocytoma,to further improve the level of diagnosis and differential diagnosis. Methods Refering to the relevant literature, retrospectively analyze multi-slice spiral CT finding of 2 cases with renal oncocytoma confirmed pathologically. Results The cases were all located in the lower part of the kidney, with about 3.8-5.2 cm in diameter, isodense and hyperdense on CT pre-contrast scanning, including 1 case, lesions with the star shaped scar; By contrast enhancement,the lesions was markedly obviously in the cortical phase, the enhancement degree was reduced and the density was homogeneous in the parenehymal phase and renal pelvis phase ,the enhancement of the star shaped scar were delayed. 2 cases of CT three-dimensional imaging showed that the tumor blood vessels originated in the anterior-inferior branch of renal arter- ies,of which 1 case was found renal arteries and veins short circuit. Conclusion The multi-slice spiral CT on renal oncocytoma has a certain characteristic, understanding these features help to improve the diagnostic accuracy;At the same time,the multi-slice CT three-dimensional vascular imaging provide important basis for clinical preoperative plan.
