负压对超声引导下甲状腺细针穿刺结果的影响

The Influence of Negative Pressure Suction on Ultrasound-guided fine-needle Aspiration of Thyroid Nodules

目的探讨负压对超声引导下甲状腺细针穿刺结果的影响。方法回顾分析2014年2月至2014年12月期间在我院接受超声检查79位甲状腺结节患者,共87个甲状腺结节。所有结节均得到手术病理或穿刺活检病理证实。在87个结节的穿刺过程中,采用目前常用的两种的穿刺方法进行分组,第一种无负压穿刺方法共41个结节,第二种有负压穿刺方法共46个结节。对照分析87个甲状腺结节细胞学及病理学结果,得出穿刺细胞学的敏感性、特异性、准确性,比较两种穿刺方法的敏感性、特异性、准确性。采用SPSS21.0统计软件分析数据,计数资料采用χ2检验,以P<0.05差异有统计学意义。结果在87个甲状腺结节中,32个为恶性结节,55个为良性结节。无负压超声引导下细针穿刺细胞学检查的敏感度、特异度、准确率分别为93.3%、88.5%、90.2%。负压超声引导下细针穿刺细胞学检查的敏感度、特异度、准确率分别为88.2%、89.6%、89.1%。经各项比较P均>0.05,统计学无差异。结论两种超声引导下甲状腺结节细针穿刺细胞学检查的方法,其敏感性、特异性、准确性经比较,P均>0.05,统计学无差异。

Objective The aim of this research was to evaluate the influence of negative pressure suction on ultrasound-guidcd fine-needle aspiration biopsy （US-FNAB） of thyroid nodules. Methods From February 2014 to December 2014, 79 patients with 87 thyroid nodules underwent Ullrasound-guided lineneedle aspiration. All patients were divided into two groups according to whether done with negative pressure suction or not. All the nodules were subsequently verified by histology or follow-up findings. The aspiration cytology sensitivity, specificity, accuracy of each group was compared. Results Among 87 thyroid nodules diagnosed by cytology, actually there were 55 benign and 32 malignant nodules confirmed by the pathological or follow-up ultrasound. The sensitivity, specificity and accuracy were determined to be 93.3%,88.5%,90.2Voin the group puncture with negative pressure, and 88.2%, 89.6Vo, 89.1% in another group. Conclusion There was no statistical difference in two methods thyroid fine needle aspiration of its sensitivity, specificity, accuracy.
