摘要
目的探讨高速涡轮钻微创去骨分牙法拔除下颌阻生智齿在基层医院应用的疗效。方法收集连州市红十字会医院2013年5月~2015年5月门诊低位埋伏第三磨牙共164例,按随机数字法分为两组:微创组82例;常规组82例。微创组应用高速涡轮钻微创去骨分牙法,常规组应用凿骨劈冠法。所有手术病例均记录手术时间,并于术后第1、3、7天复查。对比分析两组患者手术时间,疼痛、张口受限、肿胀程度,淤血、皮下气肿和干槽症发生率的情况。结果微创组平均手术时间低于常规组,p〈0.05。微创组术后第1、3天,疼痛、肿胀程度均轻于常规组,p〈0.05,而术后第7天两组无明显差异,p〉0.05。微创组术后第1,3,7天张口受限程度均轻于常规组,p〈0.05。干槽症、淤血、皮下气肿术后发生率的对比,两组无显著性差异,p〉0.05。结论微创去骨分牙法除下颌阻生智齿可以缩短手术时间,减轻术后并发症,可在基层医院推广应用。
Objective To study the curative effect analysis of extraction of impacted wisdom teeth by high speed turbine bodkin minimally invasive boned bicuspidization in basic hospital. Methods 164 outpatient service cases of Low ambush third molars were collected at The Red Cross Hospital of Lianzhou City from May 2013 to May2015. All the cases were randomly divided into two groups,minimally invasivation group( MI group) including 82 cases and Conventional surgery group( CS group) including 82 cases. Minimally invasive boned bicuspidization was choosed in MI group,and traditional chisel was choosed in CS group. The duration of operations were recorded in all cases. Followed out- patient review at 1,3,7 day after operation. We compared the duration of operation,pain,limitation of mouth opening,swelling degree,and the incidence rates of alveolities and congestion and subcutaneous emphysema between two groups. Results The average duration of operation of MI group was less than CS group( P〈0. 05). The degree of pain and swelling of MI group was less than CS group at 1,3 day after operation( P〈0. 05),but no difference at the 7 day after operation( P〉0. 05). The degree of mouth opening limitation of MI group is less than CS group at 1,3,7 day after operation. The incidence rates of alveolities congestion and subcutaneous emphysema were no difference between two groups( P〉0. 05). Conclusion Extraction of impacted wisdom teeth by minimally invasive boned bicuspidization can shorten the duration of operation,and reduce the postoperative complications,and can be applied in basic hospitals.
