社区护理干预在老年糖尿病患者中的应用效果

Effect of Community Nursing Intervention on Elderly Patients with Diabetes Mellitus

目的探讨社区护理干预在老年糖尿病患者中的应用效果。方法选取2014年1月—2015年1月来该院治疗的老年糖尿病患者262例,随机分为两组,在院期间对两组患者均进行系统的健康教育,出院后仅对实验组采用社区护理干预方法。出院后随访1年,统计记录两组患者出院时及随访结束时的空腹血糖和餐后2 h血糖,并比较两组患者随访期间的遵医情况。结果出院时实验组的空腹血糖和餐后2 h血糖与对照组相比无明显差异,差异无统计学意义(P>0.05);实验组的空腹血糖为(6.28±1.53)mmol/L、餐后2 h血糖为(7.67±0.82)mmol/L明显优于对照组的(8.42±1.63)mmol/L和(9.18±1.13)mmol/L,差异有统计学意义(P<0.05)。实验组随访期间的遵医情况,包括定期复查,服药情况,自我监测,以及运动疗法方面均明显优于对照组,差异有统计学意义(P<0.05)。结论对老年糖尿病患者采取社区护理干预,能明显降低空腹血糖和餐后2 h血糖,并且能提升患者的遵医行为,值得临床推广。

Objective To explore the effect of community nursing intervention on elderly patients with diabetes mellitus.Methods Select January 2014-January 2015 for the treatment of elderly patients in our hospital 262 cases, were randomly divided to two groups. In the hospital during the period of two groups of patients were given systematic health education.After discharge only in the experimental group the community nursing intervention methods. After 1 years of follow-up, the fasting blood glucose and postprandial 2 h blood glucose were recorded in the two groups, and compared with the two groups of patients during the follow-up period. Results No test group of fasting plasma glucose and postprandial glucose 2 h significant difference compared with the control group, a statistically significant discharge（P〈0.05）.Fasting blood glucose in the experimental group（6.28±1.53） mmol/L, 2 h postprandial blood glucose（7.67±0.82） mmol/L was significantly better than the control group（8.42±1.63） mmol/L and（9.18±1.13） mmol/L,The difference was statistically significant（P〈0.05）.Compliance cases in the experimental group during follow-up, including regular review, medication compliance, self-monitoring, and sports therapies were significantly better than the control group, the difference was statistically significant（P〈0.05）. Conclusion Community nursing intervention in elderly patients with diabetes can significantly reduce the fasting blood glucose and postprandial 2 h blood glucose, and can improve the patient＇s compliance behavior, it is worthy of clinical promotion.
