两种型全口义齿在重度牙槽嵴吸收患者临床修复效果的比较

Comparison of clinical performance between two types of occlusion of complete dentures

目的:比较解剖式全口义齿和舌向集中全口义齿的临床修复效果。方法:选择2012-2013年来我院修复科就诊的重度牙槽嵴吸收患者62例,分为2组,分别采用解剖式型和舌向集中式型制作全口义齿,分别在3、6、12个月时采用吸光度法对两组的咀嚼效率进行检测,3个月时患者填全口义齿满意度调查表,对美观程度、咀嚼功能、固位功能、语音功能、舒适性等5项内容进行评分。采用SPSS12.0软件对检测结果进行统计学分析。结果:无论在3个月时、6个月时、12个月时,在相同的咀嚼时间内,舌向集中全口义齿的咀嚼效率均明显优于解剖式全口义齿(P<0.05),在满意度方面,两组的咀嚼功能和固位功能的满意度差异显著(P<0.05),其余方面无差异。结论:对于牙槽嵴重度吸收的牙列缺失患者,应用舌向集中式全口义齿,咀嚼效率、固位稳定性均能够得到明显提高,患者较满意。

Objective： To compare the clinical performance between complete dentures with anatomic and lingualized occlusion. Methods： 62 patients were divided into two groups： anatomic occlusion for the control group, and lingualized occlusion for the experiment group. The masticatory function was evaluated when wearing the complete dentures for 3, 6 and 12 months. 3 months after complete denture delivery, esthetics, masticatory function, denture retention, patient phonetics and comfort were evaluated. Results： There was significant difference between lingualized occlusion and anatomical occlusion on the masticatory function（P〈0.05）. Significant differences in retention and masticatory function were found between the lingulized and anatomic occlusal dentures（P0.05）. Conclusion： Complete denture with lingualized occlusion could improve the masticatory function and stability of patients with severe alveolar ridge absorption.
