超声引导下巴德穿刺枪在肾活检的临床应用分析

目的探讨彩色多普勒超声引导巴德活检枪在肾活检中的临床应用价值。方法选择2011年3月—2012年12月行彩超引导下斜角进针巴德(BARD)穿刺枪肾活检术86例患者和1996年11月—1998年1月彩超定位负压吸引肾活检术的80例患者作为研究对象,对两种穿刺方法的成功率及并发症的发生率进行回顾性分析。结果BARD活检枪组,第一次穿刺成功率94.19%(81/86),第二次成功率97.67%(84/86),第三次成功率100%,活检标本平均长度(14.4±4.6)mm,肾小球平均数目(22.8±10.5)个,肉眼血尿发生率为2.33%(2/86),肾包膜下血肿发生率2.33%(2/86),腰痛发生率9.30%(8/86),并发症发生率为13.9%;负压吸引活检针组第一次穿刺成功率90.00%(72/80),第二次成功率92.50%(74/80),第三次成功率100%,活检标本平均长度(11.4±3.1)mm,肾小球平均数目(14.5±8.2)个,肉眼血尿发生率2.50%(2/80),肾包膜下血肿发生率2.50%(2/80),腰痛发生率为11.25%(9/80),并发症发生率为16.25%,2组比较差异均有统计学意义(P<0.05)。结论彩色多普勒超声引导巴德穿刺活检枪行肾活检术具有操作简便、成功率高、取材佳、并发症少等优点,值得广泛推广。

Objective To discuss the clinical application of ultrasound-guided BARD biopsy gun in the renal biopsy. Methods The successful rateand the rate of complications were retrospectively analyzed in 86 cases of ultrasound-guided Bard gun renal biopsy from March 2011 to December 2012 and 80 cases of ultrasound-negative pressure biopsyfrom November 1996 to January 1988 admitted inour hospital. Results Once successful biopsy rate was 94.19% ,twice success- ful biopsy rate was 97.67 % ,third successful biopsy rate was 100% ,the average length of renal sample was（ 14.4 ±4.6） millimeter, the average number of glomeruli was 22.8 ± 10.5, the rate of gross hematuria was 2.33%, renal minor hematoma was 2.33% and backache was 9.30% , the rate of complications 13.9%, respectively in the former group and 90.00% ,92.50%, 100%, （ 11.4 ± 3.1 ） millimeter, 14.5 ± 8.2,2.50% ,2.50% and 11.25%, the rate of complications 16.25%, respectively in the latter group. Part of the difference in the two groups was statistically significant. Conclusion Ultrasound-guided BARD biopsy gun is not only easy to practice, but also has higher-quality drawn with low occurrence of complications.
