调肝汤口服加热敷治疗慢性盆腔炎随机平行对照研究

Tiaogan Soup Treatment Chronic Pelvic Inflammatory Disease Randomized Controlled Study

[目的]观察调肝汤口服加热敷治疗慢性盆腔炎疗效。[方法]使用随机平行对照方法,将147例门诊患者按就诊顺序号法简单随机分为两组。对照组73例青霉素,640万u/次,2次/d,静滴;左氧氟沙星0.3g+0.9%500m L生理盐水,1次/d,静滴;甲硝唑,250m L,1次/d,静滴;金刚藤,2g/次,3次/d,口服。治疗组74例调肝汤:山药淮、阿胶珠、当归炒10g,白芍炒20g,山萸肉、巴戟天各10g,甘草3g;脾肾两虚-湿热阻滞合易黄汤(山药、芡实各30g,黄柏6g,车前子3g,白果12g);脾虚湿盛-气血亏虚合完带汤(白术、山药各30g,人参6g,白芍15g,车前子、苍术各9g,甘草3g,陈皮、黑芥穗、柴胡各2g);肝肾阴虚-气滞血瘀合宣郁通经汤(白芍酒炒、当归酒洗、丹皮各15g,山栀炒9g,白芥子炒研6g,柴胡、香附酒炒、川郁金醋炒、黄芩酒炒、生甘草各3g);1剂/d,水煎300m L,早晚口服,150m L/次;药渣加醋调匀,热敷下腹。连续治疗10d为1疗程。观测临床症状、症候积分、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈39例,显效17例,有效13例,无效5例,总有效率93.24%。对照组痊愈12例,显效15例,有效27例,无效19例,总有效率73.97%。治疗组疗效优于对照组(P<0.01)。症候积分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]调肝汤口服加热敷治疗慢性盆腔炎疗效满意,无严重不良反应,值得推广。

[Objective]To observe Tiaogan soup treatment of chronic pelvic inflammatory effect.[Methods]Randomized parallel controlled method,147 cases of outpatient treatment order number method is simple and randomly divided into two groups.The control group 73 cases of penicillin,6.4 million u/times,2 times/d,intramuscular injection.Levofloxacin 0.3g＋0.9% 500 m L saline,1 times/d,intravenous infusion.Metronidazole,250 m L,1 times/d,intravenous infusion.Jingangteng,2g/time,3 times/d,orally.Treatment group 74 cases of Tiaogan soup,Huaishanyao,A’jiaozhu,Danggui10 g,Baishao 20 g,Shanyurou,Bajitian each 10 g,Gancao 3g;spleen and kidney deficiency-heat block All Easy yellow soup（Shanyao,Qianshi each 30 g,Huangbai 6g,Zheqianzi 3g,Baiguo 12g）;spleen wet Sheng-qi deficiency combined with complete soup（Baizhu,Shanyao 30 g,Renshen 6g,Baishao 15 g,Cheqianzi,Cangzhu 9g,Gancao 3g,Chepi,Heijieshui,Chaihu 2g）;liver and kidneyblood stasis combined Xuan Yu pass through the soup（Baishaojiuchao,Danguijiuxi,15 g,Shanzhichao 9g,Baijiezichaoyan6 g,Chaihu,Xiangfujiuchao,Chuanyujincuchao,Huangqinjiuchao,Shenggancao 3g）;1/ d,decoction 300 m L,sooner or later,oral,150 m L/second;medicine slag vinegar and mix thoroughly,hot abdomen continuous 10 d for a course of treatment.observation of clinical symptoms,adverse reactions.2 courses of continuous treatment,to determine efficacy.[Results]The cure 39 cases,17 cases markedly effective in 13 cases,5 cases,the total efficiency of 93.24% in the control group cured 12 cases,15 cases markedly effective in 27 cases,19 cases,the total efficiency of 73.97%.treatment group than the control group（P〈0.01）.syndrome score in both groups improved（P〈0.01）,the treatment group than the control group improved（P〈0.01）.[Conclusion]Tiaogan soup treatment satisfaction chronic pelvic inflammatory disease curative effect,no serious adverse reactions,is worth promoting.
