腰椎椎体终板改变与椎间盘退变的相关性研究

THE CHANGES OF LUMBAR END-PLATE AND LUMBAR DISC DEGENERATION:A CORRELATION STUDY

目的探讨腰椎椎体终板角与腰椎间盘退变的关系。方法选取于本院行腰椎MRI检查的无特殊病史和病变的病人316例,男186例,女130例。在MRI正中矢状位T2加权像上按PFIRRMANN分级确定L3-5椎间盘的退变程度,测量L3-5椎间盘相邻椎体终板的终板角,并分析二者相关性。结果 L3-5椎间盘相邻椎体终板的终板角在男女间差异无显著性（P〉0.05）。L3-5椎间盘相邻椎体终板角与椎间盘退变程度呈显著正相关（r=0.952～0.977,P〈0.01）。结论椎体终板角与腰椎间盘退变程度呈正相关,MRI正中矢状位T2加权像终板角度的测量在一定程度上反映了腰椎间盘的退变程度。

Objective To investigate the relationship between angle of vertebral endplate of lumbar spine and lumbar disc degeneration. Methods A total of 316patients（186males,130females）who received MRI examination with no specific history or disease were selected in this study.The concave angles of vertebral endplate were measured from inferior surface of L3 to superior of L5 in the median sagittal T2-weighted MR images.The degree of L3-5intervertebral disc degeneration was observed according to the PFIRRMANN grading,and the correlation of concave angle of vertebral endplate with intervertebral disc degeneration were analyzed. Results There was no significant difference between male and female in L3-5concave angle of vertebral endplates（P〈0.05）.L3-5discs adjacent concave angle of vertebral endplate was positively correlated with the degree of intervertebral disc degeneration（r=0.952-0.977,P〈0.01）. Conclusion Vertebral endplate angle is positively correlated with the degree of lumbar disc degeneration,the measurement of endplate angle in median sagittal T2-weighted MR images reflects the degree of degeneration of lumbar disc to some extent.
