不同术式对膝骨关节炎患者近期预后影响分析

Analysis of the Influence on the prognosis in patients with osteoarthritis by different operative methods

目的分析传统膝关节置换术与髌骨整形术对骨关节炎患者近期预后的影响,为临床治疗提供参考依据。方法将73例骨关节炎患者根据手术方式不同分为两组,观察组39例行髌骨整形术治疗,对照组34例行传统膝关节置换术治疗,对比两组临床效果及并发生情况。结果两组患者围术期情况、美国西部骨关节炎指数(WOMAC)评分、KSS评分、关节活动度及术后力线对位不良度数、胫骨倾斜度方面比较,差异均无统计学意义(P>0.05);观察组术后并发症发生率5.13%,明显低于对照组的23.53%(P<0.05)。结论传统膝关节置换术与髌骨整形术均可有效改善骨关节炎患者预后,但传统膝关节置换术并发症发生率高于髌骨整形术,值得临床重视。

Objective To analyze the influence of the traditional knee joint replacement and patellar plasty on prognosis of patients with osteoarthritis. Methods Seventy- three cases of osteoarthritis patients were divided into two groups according to the operation methods. 39 patients in the observation group were given patellar plasty and 34 in the control group underwent traditional knee joint replacement. The clinical response was compared between the two groups. Results There was no statistical difference in the perioperative period,WOMAC score,KSS score,joint range of motion（ ROM） and postoperative tibial slope and the lines of force of dislocation degree between the two groups（ P 〉0. 05）. The incidence rate of complications in the observation group was 5. 13%,significantly lower than that in the control group,which was 23. 53%（ P 〈0. 05）. Conclusion Both traditional knee joint replacement and patellar plasty can effectively improve the prognosis of patients with osteoarthritis,but more complications are reported in the patients treated by traditional knee joint replacement than patellar plasty patella,which is worthy of attention.
