慢性阻塞性肺疾病患者外周血T淋巴细胞凋亡检测的临床价值 被引量:4

摘要 目的探讨慢性阻塞性肺疾病(COPD)患者外周血CD4+、CD8+T细胞凋亡水平与肺功能的相关性。方法采用流式细胞术测定30例COPD缓解期患者、30例COPD急性期患者及20名健康体检者外周血CD4+和CD8+T细胞凋亡水平,与第1秒用力呼出量占预计值百分比(FEV1%)和FEV1/用力肺活量(FVC)水平作相关分析。结果 COPD患者CD4+T细胞凋亡水平较健康对照组明显升高(P<0.01),其中急性期显著高于缓解期(P<0.05);COPD患者CD8+T细胞凋亡水平明显低于健康对照组(P<0.01),其中急性期明显低于缓解期(P<0.05)。CD4+T细胞凋亡率与FEV1%、FEV1/FVC呈负相关(P<0.01);CD8+T细胞凋亡率与FEV1%、FEV1/FVC呈正相关(P<0.01)。结论 CD4+、CD8+T细胞凋亡水平与肺功能具有密切相关性,其水平变化可作为监测COPD患者病情的重要指标。 Objective To investigate the correlation of the apoptosis of peripheral blood CD4+and CD8+T cells with lung function in chronic obstructive pulmonary disease( COPD) patients. Methods A total of 30 stable COPD patients,30 acute exacerbation COPD patients and 20 healthy subjects were enrolled. The apoptosis of peripheral blood CD4+and CD8+T cells was determined by flow cytometry. The correlation of T lymphocyte apoptosis with forced expiratory volume 1 second( FEV1%) and FEV1 / forced vital capacity( FVC) was analyzed. Results The CD4+T cell apoptosis of COPD group increased significantly with that of healthy control group( P 〈 0. 01),and that of acute exacerbation group was higher than that of stable group( P 〈 0. 05). The CD8+T cell apoptosis of COPD group was lower than that of healthy control group( P 〈 0. 01). The CD8+T cell apoptosis of acute exacerbation group was lower than that of stable group( P 〈 0. 05). CD4+T cell apoptosis was negatively correlated with FEV1% and FEV1 / FVC,and CD8+T cell apoptosis was positively correlated with FEV1% and FEV1 / FVC( P 〈 0. 01). Conclusions The apoptosis of CD4+and CD8+T cells is closely related to lung function in COPD patients,which can be used as an indicator for the monitor of COPD.
