前入路绕肝提拉法联合Glisson蒂横断在半肝切除术中的临床应用

Application of Liver Hanging Maneuver Combined with Glissonean Pedicle Transection During Anterior Approach Half Hepatectomy

探讨前入路绕肝提拉法联合Glisson蒂横断在半肝切除术中的应用价值。方法回顾性分析我院2009年1月至2013年12月16例采用前入路绕肝提拉联合Glisson蒂横断术行肝脏切除患者的资料。结果术中出血量少,无手术中输血,手术时间短,术后肝功能恢复快,全部病例均未出现肝功能不全,无手术死亡。1例出现胸腔积液,予以穿刺抽液后好转。发生胆漏1例,予以引流后治愈。肿瘤患者的1年生存率为78.6%(11/14)。结论前入路绕肝提拉法及Glisson蒂阻断技术联合应用,能减少术中出血,防止术后并发症,减少肝脏缺血再灌注,避免术中挤压肝脏引起的肿瘤转移,值得临床推广应用。

Objective To evaluate the value of liver hanging maneuver combined Glissionean pedicle transaction in the half hepatectomy. Methods The clinical data of 16 patients who underwent the half hepatectomy using liver hanging maneuver combined Glissionean pedicle transaction in our hospital were retrospectively analyzed. Results The blood loss were little during the operation and there were no blood transfusion during operation. The operation time were short and liver function recovered quickly after operation. There were no liver or renal dysfunction, no death caused by operation occurred. 1case of pleural effusion was recovered by puncture liquid. Conclusion The application of liver hanging maneuver combined Glissionean pedicle transaction in the half hepatectomy might be safe and a effective method to decrease hemorrhage and prevent postoperative liver dysfunction.
