Wiltse入路结合TLIF技术治疗峡部不连性腰椎滑脱21例临床分析 被引量:1

摘要 目的探讨Wiltse入路下峡部不连性滑脱的手术效果及优势。方法对21例经Wiltse入路结合椎间孔腰椎椎体间融合术(TLIF)治疗的峡部不连性滑脱患者临床资料进行回顾性分析,统计其手术前后JOA评分、疼痛视觉模拟评分(VAS)及Oswestry功能障碍指数(ODI);观察手术时间、出血量和滑脱复位效果及融合效果,并对部分患者术后多裂肌萎缩指数进行分析。结果手术时间150~300(205.23±40.45) min;出血量150~600(336.37±117.32) Ml ;手术前后的JOA评分、VAS及ODI比较的差异均有统计学意义(P<0.001);影像学上滑脱复位效果满意,随访时融合节段均达到临床融合标准;多裂肌萎缩指数为0.83±0.15。结论 Wiltse入路结合TLIF治疗可对峡部不连性腰椎滑脱进行良好减压、复位及融合,并在防止多裂肌萎缩方面有明显优势。
