规范化癌痛治疗的疗效及对癌症患者及家属生活质量影响

Efficacy of Standardized Treatment of Cancer Pain For Cancer Patients and Their Families

目的研究规范化癌痛治疗的疗效,以及癌痛控制患者和家属的心理状态及生活质量。方法对346例中、重度癌痛患者严格按WHO三阶梯止痛治疗,用数字评分法对患者疼痛进行全面评估,了解癌症患者72 h的疼痛控制率,以及癌痛控制后癌症患者及家属的心理状态及生活质量改变。结果规范化癌痛治疗72 h后患者疼痛明显缓解,其中中度疼痛患者明显缓解率为96.02%,重度疼痛患者明显缓解率为93.10%。规范化癌痛治疗后,患者的抑郁、焦虑、恐惧、消极情绪均得到了明显改善(P<0.05),自杀倾向率显著降低(P<0.05),对疾病治疗的自信心显著提高(P<0.05)。同时患者家属的抑郁、焦虑、恐惧、消极情绪也得到了明显改善(P<0.05)。生活质量上,患者的睡眠、精神状态、日常生活能力均明显好转(P<0.05),食欲状态改善无显著差异(P>0.05);而患者家属的睡眠、食欲、精神状态及日常生活能力4个方面均明显好转(P<0.05)。结论规范化癌痛治疗能在短时间内迅速控制疼痛。癌痛控制后,患者及患者家属的心理状态得到明显改善,生活质量得到显著提高。

Objective To evaluate the clinical efficacy of standardized treatment of cancer pain for cancer patients and their families,and its effect on mental state and quality of life.Methods 346 cases of moderate and severe cancer pain patients received standardized treatment of cancer pain.Each pain was evaluated by standardized treatment in 72 h,and to evaluate the change of psychological state and quality of life of cancer patients and their families.Results Standardized treatment of cancer pain could control cancer pain significantly after 72 h.Pain relief was 96.02% in moderate cancer pain and 93.10% in severe cancer pain.And depression,anxiety,fear,negative emotions of patients have been significantly improved（P〈0.05）,suicide rate was significantly lower（P〈0.05）,and the self-confidence of treatment of diseases improved significantly（P〈0.05）.Meanwhile depression,anxiety,fear,negative emotions of the families of patients have also been significantly improved（P〈0.05）.The quality of life,sleep,mental status,activities of daily living in patients were significantly improved（P〈0.05）,but appetite status had no significant difference（P〈0.05）.The sleep,appetite,mental state and ability of the families were significantly improved（P〈0.05）.Conclusion The cancer patients and families can benefit from the standardized pain therapy.
