核磁共振成像参数对颈椎轻微创伤后急性脊髓损伤风险的评估 被引量:3

摘要 目的研究核磁共振成像(magnetic resonance images,MRI)各参数对颈椎轻微创伤后急性脊髓损伤(spinal cord injury,SCI)的预测及严重程度评估。方法对2003年1~8月确诊轻微颈椎创伤后患者传统侧位X线片及T2加权MRI图像进行回顾性分析,MRI中计算颈椎管率、椎管有效空间及椎管脊髓比值,传统X线片中计算颈椎管率。ROC曲线用于评估各个参数对于SCI风险预测、敏感性及其进展的精确分类。结果 SCI组所有MRI参数值均小于对照组,差异均有统计学意义(P〈0.05)。不同AIS等级SCI患者所有影像学参数之间差异均无统计学意义(P〉0.05)。椎间盘水平椎管直径最小为8 mm,对于预测轻微颈椎创伤后SCI的发生具有最高的阳性预测值(84%)及阳性似然比(15.6)。椎间盘处椎管最小直径(8 mm)ROC曲线以下区域与其他参数相比最大。结论 MRI中轻微颈椎创伤后患者椎间盘水平椎管直径小于8 mm时,发生SCI的风险急剧增加。影响影像学参数的各种原因(如创伤机制、神经细胞受损程度等)都可能影响SCI的严重程度。
