摘要 目的探讨早期康复结合低频电刺激对急性脑卒中偏瘫患者运动功能和生活质量的影响。方法 124例急性脑卒中患者随机分为对照组和观察组各62例,对照组进行早期康复干预;观察组进行早期康复结合低频电刺激干预,20d为一个疗程,治疗2个疗程后评估神经功能缺损恢复情况;采用简式Fugl-Meyer运动功能积分法评估患者的肢体运动功能,Barthel指数评估日常生活活动能力;Ashworth量表评定肌张力,应用GQOLI生活质量综合问卷评估生活质量。结果干预后,观察组上肢、下肢Fugl-Meyer和Ashworth肌张力0~Ⅰ级均高于对照组,Ashworth肌张力Ⅳ级低于对照组,差异均有统计学意义(P〈0.05);观察组神经功能缺损低于对照组,Barthel指数和生活质量评分高于对照组,差异均有统计学意义(P〈0.05)。结论早期康复结合低频电刺激能显著改善急性脑卒中偏瘫患者运动功能,并提高患者的生活质量。 Objective To understand the effect of early rehabilitation plus low frequency electrical stimulation in restoring the movement function and life quality of patients with hemiplegia resulting from acute stroke. Methods 124 patients of acute cerebral stroke were randomly divided into a control group( n = 62) and an experimental group( n = 62). The control group was administered with early rehabilitation intervention while the experimental group was treated with low frequency electrical stimulation in addition to early rehabilitation intervention. Both groups were subjected to two sessions of treatment,each consisting of 20 days. Then the recovery of the nervous function of both groups was assessed. The simplified form of Fugl-Meyer movement function integral method was used to assess the limb movement function. Barthel index was used to evaluate the ability to live a normal life. Ashworth scale was used to assess the muscle tone. GQOLI life quality comprehensive questionnaire was used to assess the quality of life. Findings After two sessions of treatment,the Fugl-Meyer and Ashworth muscle tones of the upper and lower limbs at level 0 ~ Ⅰ of the experimental group were higher than those of the control group. The difference was statistically significant( P〈0. 05). The Ashworth muscle tone of the experimental group at level IV was lower than that of the control group. The difference was statistically significant( P〈0. 05). The defection rate of nervous function of the experimental group was lower than that of the control group. Thedifference was statistically significant( P〈0. 05). The Barthel index and quality of life scores of the experimental group were higher than those of the control group. The difference was statistically significant( P〈0. 05). Conclusion Early rehabilitation plus low frequency electrical stimulation can significantly improve the nervous function,the movement function,and the quality of life of patients of hemiplegia resulting from acute cerebral stroke.
