Experience of YAO Nai-li in the treatment of chronic atrophic gastritis based on 'collaterals disease theory'

文章总结了姚乃礼基于络病理论论治慢性萎缩性胃炎(CAG)的经验。提出了以"脾虚络阻毒损"为本病的基本病机,从补益脾胃以固本、活血通络以驱邪、解毒散结以防变3个方面辨治,据此确立了"健脾通络解毒"的治疗法则,设立"健脾通络解毒方"为治疗CAG基本方。治疗时重视胃镜像表现,宏观与微观辨证相结合,遣方用药,从而有效治疗CAG,延缓癌变趋势。

Based on ‘collaterals disease theory＇, this paper summarized the experience of YAO Nai-li in the treatment of chronic atrophic gastritis（CAG）. In this paper, ‘spleen deficiency, collateral stagnation and toxin damaging＇ were considered as the basic pathogenesis of the disease. Here was differently dealing with the three pathogenic factors. Firstly, tonifying the spleen and stomach was used to consolidate the constitution. Secondly, activating blood and regulating the collaterals were used to dispel pathogenic factors. Thirdly, detoxicating and dispersing accumulation of pathogen were used to prevent from a progress of the disease. Finally, a principle as well as a main plan that strengthening the spleen, regulating the collaterals and detoxicating was set up in the treatment of CAG. Meanwhile, it indicated that we should pay high attention on results of the gastroscope image in the processes of the differentiation. The reason was that only when we combined macroscopic syndrome differentiation with microcosmic syndrome differentiation could we reach perfect goals that CAG could be effectively treated and the tendency of cancerization could be delayed.
