泻火止痛方治疗肝火上炎型偏头痛的临床研究

Clinical Observation on Xiehuo Zhitong Decoction in Treatment of Migraine with Liver-fire Ascending Syndrome

目的观察泻火止痛方治疗肝火上炎型偏头痛的临床疗效。方法选择符合偏头痛诊断标准,且属于中医肝火上炎证型的患者108例,随机分为治疗组(55例)与对照组(53例)。治疗组予泻火止痛方颗粒,每日1包,分2次口服;盐酸氟桂利嗪胶囊模拟剂5mg,每晚1次口服。对照组予泻火止痛方颗粒模拟剂,每日1包,分2次口服;盐酸氟桂利嗪胶囊5mg,每晚1次口服。两组疗程均为8周。观察治疗前后两组临床疗效、中医证候疗效、偏头痛积分、中医证候积分。结果治疗组临床疗效总有效率为87.27%,对照组总有效率为79.25%,两组比较差异有统计学意义(P<0.01)。两组治疗后偏头痛积分较治疗前均有下降(P<0.01),偏头痛积分改善情况治疗组优于对照组(P<0.01)。治疗组中医证候疗效总有效率为89.09%,对照组总有效率为75.47%,两组比较差异有统计学意义(P<0.05);两组在治疗后中医证候积分较治疗前均有下降(P<0.01),中医证候积分改善情况治疗组优于对照组(P<0.01)。结论泻火止痛方治疗辨证属于肝火上炎证型的偏头痛,具有良好的临床疗效。

Objective To investigate the clinical effect of Xiehuo Zhitong decoction（XZD）in treating migraine with liver-fire ascending syndrome.Methods One hundred and eight patients with migraine and liver-fire ascending syndrome were randomly divided into two groups：Control group（n=53）treated by flunarizine hydrochloride plus simulation agents of XZD,and treatment group（n=55）treated by XZD plus simulation agents of flunarizine hydrochloride for 8 weeks.The total effective rate,the effective rate of symptom score,scores of migraine and the symptom scores of traditional Chinese medicine（TCM）were observed.Results After 8 weeks of treatment,the total effective rate was 87.27%in treatment group and 79.25%in control group（P〈0.01）.The scores of migraine were improved after treatment in both group,with more significant improved in treatment group than in control group（P〈0.01）.The effective rate of symptom score was 89.09%in treatment group and 75.47%in control group（P〈0.05）.The symptom scores were improved after treatment in both group,with more significant improved in treatment group than in control group.ConclusionXZD is quite effective in treating migraine with liver-fire ascending syndrome.
