摘要
目的探讨宫颈癌、宫颈上皮内瘤变(CIN)组织增殖细胞核抗原(PCNA)、Ki-67表达水平及其与人乳头瘤病毒(HPV)感染的关系。方法选择接受手术治疗的宫颈病变患者140例,其中宫颈癌36例,CIN74例,以慢性宫颈炎30例作为对照组。采用免疫组织化学S-P法检测宫颈组织标本PCNA、Ki-67表达情况,采用聚合酶链反应检测宫颈组织HPV感染情况。结果宫颈癌组、CIN组PCNA、Ki-67表达阳性率高于对照组,宫颈癌组、CINⅡ~Ⅲ组PCNA、Ki-67阳性率高于CINⅠ组(P〈0.05)。宫颈癌组、CIN组PCNA、Ki-67过表达检出率高于对照组,CINⅡ~Ⅲ组PCNA、Ki-67过表达检出率高于CINⅠ组(P〈0.05)。PCNA表达强度与HPV感染呈正相关(相关系数为0.706,P〈0.05),Ki-67表达强度与HPV感染呈正相关(相关系数为0.695,P〈0.05)。结论早期宫颈癌及不同CIN分期病变组织PCNA、Ki-67表达水平差异较大,不同CIN分期病变组织HPV感染率亦存在较大差异,可联合检测HPV、PCNA、Ki-67等指标对早期宫颈癌、CIN进行辅助诊断。
Objective To study the expression of proliferating cell nuclear antigen(PCNA)and Ki-67 in cervical cancer and cervical intraepithelial neoplasia(CIN),and the correlation with human papilloma virus(HPV)infection.Methods A total of 140 patients,receiving cervical resection,were enrolled,including 36 cases of cervical cancer,74 cases of CIN,and 30 cases of chronic cervicitis as control group.Immunohistochemical detection of PCNA and Ki-67,and polymerase chain reaction detection of HPV were performed in cervical specimens.Results Positive rate of PCNA and Ki-67 in cervical cancer group and CIN group were significantly higher than control group,and those of CIN Ⅱ-Ⅲ group were higher than CINⅠ group(P〈0.05).In cervical cancer group and CIN group,detection rates of over-expression of PCNA and Ki-67 were significantly higher than control group,and those of CINⅡ-Ⅲ group were higher than CINⅠ group.The expression of PCNA and Ki-67 were positively correlated with HPV infection,with correlation coefficient of 0.706 and 0.695(P〈0.05).Conclusion There could be difference of the expression of PCNA and Ki-67 between cervical cancer and different levels of CIN,and in different levels of CIN,the HPV infection rate might be also different.HPV,PCNA,Ki-67 and other indicators should be combined detected for the diagnosis of cervical cancer and CIN.
