动态脑电地形图变化在急性进展型脑梗死中的应用

摘要 目的动态观察脑电地形图(BEAM)在脑梗死急性期的进展变化并对可能进展的脑梗死及时给予药物干预及判断预后。方法对2012年8月~2015年7月收入我科的脑梗死患者在12 h之内特别是病情有可能进展和已经进展者,进行脑电地形图检查,按国际10-20系统放置电极,用单极导联和双极导联进行持续描记,判断梗死部位及脑电地形图的异常程度。结果动态观察BEAM呈进行性加重的30例,符合条件给予溶栓并扩容、降纤、抗凝治疗后,BEAM明显好转的26例,好转的4例。结论脑电地形图局灶性异常的程度一般与临床异常体征相一致。如果是进行性脑梗死,脑波的变化比临床症状先出现。 Objective To observe the changes of brain electrical activity(BEAM)during the acute phase of cerebral infarction,and to provide the intervention and prognosis of the patients with cerebral infarction. Methods The patients with cerebral infarction from August 2012 to July 2015 within 12 hours,especially in the condition of the disease could progress and had been made,and the use of the 10-20 system to the electrodes,using unipolar lead and bipolar lead to carry on the record,to determine the extent of the infarction site and brain electrical topographic map. Results The dynamic observation of BEAM was carried out in 30 patients,with the conditions given in the conditions of thrombolysis and expansion,lowering the fiber,the BEAM was significantly improved in 26 cases,4 cases improved. Conclusion The extent of focal abnormalities of brain electrical activity map is consistent with the clinical symptoms. If there is a cerebral infarction,brain changes than the clinical symptoms first appear.
