甘露聚糖肽预防手术术后并发感染的荟萃分析

Meta-analysis of Prevention Value of Mannatide on Postoperative Infection

目的系统评价甘露聚糖肽预防手术后并发感染的效果。方法电子检索Cochrane图书馆、MEDLINE、EMBASE、CNKI、CBM,检索日期至2013年2月。纳入甘露聚糖肽预防手术术后感染的临床试验,制订严格的纳入、排除标准,纳入试验研究、评价文献质量、提取数据,并使用Rev Man 5.0软件进行Meta分析。结果纳入11个随机对照试验,结果显示甘露聚糖肽有利于降低患者术后感染的发生率(82.9%比77.4%,OR=1.47,95%CI 1.06~2.04,P=0.02),缩短患者住院时间(均数差为1.37,95%CI-1.68^-1.06,P<0.01),减少患者并发感染的可能(7.6%比20.6%,OR=0.31,95%CI 0.22~0.43,P<0.01)。结论甘露聚糖肽可减少患者术后可能发生的感染,缩短住院时间,其机制可能与提高人体免疫有关。

Objective To evaluate clinical effect of mannatide in the prevention of postoperative infec- tion. Methods Literatures were retrieved from Cochrane Library, MEDLINE, EMBASE, CNKI, CBM data- bases by computer to February 2013. Trial selection,quality evaluation and information extraction were per- formed according to including and excluding criteria. Meta-analysis was conducted using RevMan 5, 0 soft- ware. Results Altogether 11 random trials were included in the meta-analysis. The results showed manatide was helpful to decrease the incidence of postoperative infection （ 82.9% vs 77. 4%, OR = 1.47,95% Cl 1.06-2. 04 ,P = 0. 02 ）, patients＇ hospitalization time was shortened （ SMD = 1. 371.37,95% CI - 1.68- - 1.06, P 〈 0. 01 ）, the possibility of concurrent infection was reduced （7.6% vs 20. 6%, OR = 0. 31, 95% CI 0. 22-0. 43 ,P 〈0. 01 ）. Conclusion Mannatide can decrease the postoperative infection rate and hospital stay time, the mechanism of which might associated with improve immunity.
