摘要
目的探讨紫杉醇联用顺铂化疗致严重神经毒性的发生机制。方法回顾分析1例非小细胞肺癌给予紫杉醇联合顺铂致严重神经毒性的临床资料。结果本例因痰血就诊,确诊为非小细胞肺癌,入院后给予紫杉醇联合顺铂常规剂量化疗,经过3个化疗周期,疗效显著,但由于该病例在化疗期间发生了严重的神经毒性,故限制了该化疗方案的继续应用。结论紫杉醇所致严重神经毒性存在个体差异,老年患者本身细胞修复能力和肝肾等器官功能下降,对使细胞毒性药物在体内的代谢减缓,毒性作用时间延长,更易出现化疗不良反应。
Objective To discuss the mechanism of severe neurotoxicity caused by paclitaxel combined with cisplatin .Methods A retrospective analysis of clinical data was pertorem about non‐small cell lung cancer case given paclitaxel combined with cisplatin and resulted in severe neurotoxicity .Result The patients came to hospital because of sputum blood and diagnosed as non‐small cell lung cancer .Then 3 weeks chemotherapy showed significant effect ,but in the chemotherapy period ,severe neurotoxicity happened and the chemotherapy regimen discontinued .Conclusion There is individual difference in neurotoxicity caused by paclitaxel .As cells’ self‐repair ability decreased ,and liver and kidney function declined ,cytotoxic drug metabolism is slow down in vivo ,Early patients are more prone to having adverse reaction in chemotherapy .
