胰腺实性假乳头状瘤14例临床诊治分析

Diagnosis and Treatment of 14 Cases of Solid Pseudo-Papillary Tumors of Pancreas

目的:探讨胰腺实性假乳头状瘤(solid pseudo-papillary tumors of pancreas)的诊断治疗经验。方法:回顾性分析2008年1月至2014年12月在兰州大学第一医院14例SPTP的临床表现、影像学检查、病理结果、治疗及预后。结果:SPTP无特异性临床表现,可为腹部胀痛不适,CT检查可见胰腺部位囊实性或实性占位性病变,14例均行手术治疗,其中胰十二指肠术2例,肿瘤局部切除3例,胰体尾切除2例,胰腺体尾部+脾脏切除3例,胰腺中段切除+胰胃吻合3例,胰颈体尾切除1例。14例肿瘤标本病理结果均具有SPTP的典型病理学表现,随访3个月至5年,均未发现肿瘤复发及转移。结论:SPTP是一种低度恶性肿瘤,治疗以手术为主,手术方式应依据术前CT和术中探查来决定,预后良好。

Objective： To summarize and assess the diagnostic and therapeutic experiences of solid pseudo-papillary tumor of pancreas（ SPTP）. Methods： Clinical manifestation,imaging examination,pathological results,treatment and diagnosis information of 14 case of SPTP treated in our hospital from Jan. 2008 to Dec. 2014 were studied retrospectively.Results： There were no specific clinical manifestations in people with SPTP who mainly presented abdominal pain or discomfort. CT scanning suggested cystic-solid or solid masses occupation seen in pancreas. All patients underwent surgical resection including 2 cases of pancreaticoduodenectomy,3 cases of local resection,3 cases of distal pancreatectomy plus splenectomy,2 cases of distal pancreatectomy,3 cases of middle pancreatectomy,and 1 case of distal and middle pancreatectomy. The histological examination revealed typical pathological presentation of SPTP. Follow-up period ranged from 3months to 5 years,and no recurrence and metastasis were found in all. Conclusion： SPTP is a kind of less aggressive tumor with good prognosis. Surgery is the first choice for its treatment. The surgical procedures should be chosen based on the finding of preoperative CT imaging and intra-operative exploration of tumor location.
