经阴道彩色多普勒超声对宫内占位性病变的诊断价值

Clinical value of translational color Doppler ultrasound in the diagnosis of intrauterine occupying lesions

目的探讨经阴道彩色多普勒超声(TVS)在宫内占位性病变诊断中的临床应用价值。方法回顾性分析本院收治的68例接受TVS检查疑似宫内占位性病变患者的临床资料,对比分析TVS与宫腔镜检查、病理学诊断结果的差异。结果 TVS宫内占位性病变无病理改变诊断率为10.29%(7/68),宫腔镜检查为14.65%(12/68),二者比较差异无显著性(P>0.05);病理学宫内占位性病变无病理改变诊断率为25.00%(17/68),与TVS比较差异具有显著性(P<0.05);以病理学诊断为金标准,TVS诊断符合率为83.61%。血流阻力指数(RI)<0.50的患者恶性宫内占位性病变发生率为66.67%(4/6),RI≥0.50、静脉血流信号、无血流信号的患者恶性宫内占位性病变发生率为1.61%(1/62),比较差异具有显著性(P<0.05)。结论 TVS诊断宫内占位性病变准确度相对较高,且无创、无痛、方便,具有较高的临床应用价值,可作为宫内占位性病变的首选诊断方法。对于RI<0.50的宫内占位性病变患者,需结合病理学进一步明确诊断。

ObjectiveTo investigate the clinical value of TVS in the diagnosis of intrauterine occupying lesions.MethodThe clinical data of 68 patients with TVS examination suspected intrauterine occupying lesions were retrospectively analyzed, and the results of TVS, hysteroscopy and pathology diagnosis were compared and analyzed.ResultThe diagnosis rate of TVS in the diagnosis of intrauterine occupying lesions with no pathological change was 10.29% （7/68）, hysteroscopy was 14.65% （12/68）, there was no signiifcant difference （P〉0.05）. The diagnosis rate of pathology in the diagnosis of intrauterine occupying lesions with no pathological change was 25.00% （17/68）, compared with TVS, the difference was signiifcant （P〈0.05）. Taking pathological diagnosis as the gold standard, TVS diagnostic accordance rate was 83.61%. The incidence of intrauterine malignant lesions of patients with RI〈0.50 was 66.67% （4/6）, incidence of intrauterine malignant lesions of patients with RI≥0.50, venous blood lfow signal, no signal of blood lfow was 1.61% （1/62）, there was signiifcant difference （P〈0.05）.ConclusionThe accuracy of TVS in the diagnosis of intrauterine occupying lesions is relatively high, and has a high clinical value and can be used as the ifrst choice for the diagnosis of intrauterine occupying lesions. For patients with RI〈0.50, the diagnosis should be conifrmed by the combination of the pathology.
