妊娠合并疱疹样脓疱病5例临床分析

Clinical analysis of 5 pregnants with impetigo herpetiformis

回顾性分析5例妊娠合并疱疹样脓疱病的临床资料,5例患者均在孕中期发病,表现为全身泛发性红斑,其上有针尖至绿豆大脓疱,伴发热、寒战等全身不适症状。其中3例有银屑病病史。外周血白细胞和中性粒细胞偏高,血钙偏低。1例患者要求终止妊娠,给予阿维A、甲氨蝶呤治疗,其余4例以抗生素加糖皮质激素或重组人II型肿瘤坏死因子受体-抗体融合蛋白或人血丙种免疫球蛋白治疗,病情好转,新生儿均健康。

The data of 5 pregnants with impetigo herpetiformis（ IH） were analyzed retrospectively. All the patients presented with widespread erythema,some of which were covered with pustules sized from needle point to mung bean. All patients had general malaise,fever and chill. Three cases had a history of psoriasis.Laboratory evaluation revealed hypocalcemia,and increased leukocytosis and neutrophilia. One patient who asked to end the pregnancy was treated with methotrexate（ MTX） and retinoid. The other four were treated with antibiotics combined with glucocorticosteroid or intravenous immunoglobulin or recombinant human tumor necrosis factor- α receptor II： IgG Fc fusion protein. All patients got improved and newborns were health.
