压缩性老年腰椎骨折术后舒适护理对缓解患者疼痛的作用

Effect of comfortable nursing after operation of senile lumbar compression fracture on relieving patients' pain

目的研究压缩性老年腰椎骨折术后舒适护理对缓解患者疼痛的作用。方法采集2013年3月~2014年9月珠海市第二人民医院收治的压缩性老年腰椎骨折患者共80例,按随机数字表法将其分为实验组和对照组,每组各40例。对照组采用常规护理方法进行院内护理,实验组采用舒适护理干预。观察两组患者术后疼痛情况、镇痛药物用量情况以及患者心理状况[焦虑自评量表（SAS）及抑郁自评量表（SDS）],同时记录两组患者护理满意度情况。结果实验组患者疼痛情况优于对照组,实验组中重度疼痛率为42.5%,对照组为72.5%,两组比较差异有统计学意义（P〈0.05）。实验组患者镇痛药物用量低于对照组,差异有统计学意义（P〈0.05）。护理后实验组SAS[（40.82±7.66）分]以及SDS[（36.14±6.23）分]评分均优于对照组[（47.16±8.17）、（43.88±7.13）分],差异有统计学意义（P〈0.05）。实验组患者满意度（97.5%）优于对照组（77.5%）,差异有统计学意义（P〈0.05）。结论舒适护理能显著缓解压缩性老年腰椎骨折患者术后疼痛,减轻患者痛苦,改善患者因术后疼痛所致的焦虑以及抑郁倾向,同时能够取得较高的护理满意度,有利于医患关系的改善,值得在临床上广泛应用。

Objective To study the effect of comfortable nursing after operation of senile lumbar compression fracture on relieving patients＇ pain. Methods Total 80 cases of patients with senile lumbar compression fracture admitted to Zhuhai Second People＇s Hospital from March 2013 to September 2014 were collected, and they were divided into the experimental group and the control group according to the random number table, each group had 40 cases. The control group was given conventional nursing care to take hospital care, the experimental group received comfortable nursing intervention. The conditions of postoperative pain, dosage of analgesic drugs and psychological status [self-rating anxiety scale（SAS） and self-rating depression scale（SDS）] of patients were observed, and nursing satisfaction of patients in the two groups was recorded. Results The condition of pain in the experimental group was better than that in the control group, the rate of moderate and severe pain in the experimental group was 42.5%, which was 72.5% in the control group, the difference between the two groups was statistically significant（P〈0.05）. The dosage of analgesic drugs in the experimental group was lower than that of control group, the difference was statistically significant（P〈0.05）. After nursing, the scores of SAS [（40.82±7.66） points] and SDS [（36.14±6.23） points] in the experimental group were better than those of control group [（47.16±8.17）,（43.88±7.13） points], the differences were statistically significant（P〈0.05）.The degree of satisfaction in the experimental group（97.5%） was better than that of control group（77.5%）, the difference was statistically significant（P〈0.05）. Conclusion Comfortable nursing can significantly relieve postoperative pain of patients with senile lumbar vertebral fracture, lighten the suffering of patients, improve postoperative anxiety and depression tendency caused by postoperative pain, at the same time, it can achieve high degree of satisfaction,which is helpful to improve the doctor-patient relationship and worthy of clinical wide application.
