北京市2010-2013年哨点医院流感样病例监测结果分析

Analysis on influenza-like illness surveillance from one sentinel hospital in Beijing during 2010-2013

目的 分析流感哨点医院-北京大学第三医院感染疾病科流感样病例数和流感抗原快速检测结果,探索本地区流感病原学变化规律,为流感的防控提供科学依据.方法 对2010-2013年该院发热门诊上报的流感样病例数、同期门诊量,以及流感病原快速检测结果进行统计,Spearman等级相关用来分析两变量相关关系的密切程度.结果 发热门诊2010-2013年门诊量累计为137 033人次,上报流感样病例22 851人次,流感样病例就诊比例为16.88％,16-24岁组7422例（32.48％）,25至59岁组12 827例（56.13％）,60岁以上组2702例（11.82％）.2010-2013年的每年冬春季节为流感样病例高峰,2012年夏季出现另一个高峰.2010年11月-2013年12月进行流感病原快速检测,共检测6727例,检测出甲型流感2020例,乙型流感907例,阳性检出率43.51％.按月比较,病原阳性检出率与流感样病例构成比有相关性（r=0.497,p=0.002）.结论 发热门诊流感样病例数和构成比的变化是流感活跃程度的参考指标.应重视和加强每年冬春季节及夏季流感的防控.

Objective To analysis the results of influenza-like illness surveillance and influenza antigen rapid detection, and explore the etiology of influenza in the region, and provide the scientific basis for the prevention and control of influenza.Methods The reported influenza-like illness number and the outpatient number of fever clinic during 2010-2013 and the influenza antigen rapid test results were collected,spearman rank correlation was used to analyze the correlation.Results the cumulative outpatient number of fever clinic were 137 033 from 2010 to 2013 and the reported influenza-like illness were 22 851（16.88%）.The group of age 16-24 was 7422 （32.48%）, the group of age 25-59 was 12 827（56.13%）, and the age over 60 was 2702（11.82%）.Winter and spring season was the peak of influenza-like illness each year during 2010-2013, and summer was another peak in 2012.6727 patients received the influenza antigen rapid test from November 2010 to December 2013, which detected 2020 influenza A cases, 907 influenza B cases,the positive detection rate was 43.51%.The positive detection was consistent monthly with influenza-like illnesses constitute proportion.Conclusions Fever clinic influenza-like illness number and the composition ratio are the reference indication of influenza activity.It is important to strengthen influenza prevention and control in influenza season of winter, spring and summer.
