大气细颗粒物污染与慢性阻塞性肺疾病气道炎症和氧化应激关系研究 被引量:13

Relationship between fine particle pollution and airway inflammation and oxidative stress of patients with chronic obstructive pulmonary diseases

摘要 目的明确大气细颗粒物(PM 2.5)污染与慢性阻塞肺疾病(慢阻肺)患者气道炎症及其氧化应激的关系。方法选择2013年8—9月在吉林大学第一医院呼吸内科确诊为慢阻肺的78例患者(市区组41例,市郊组37例)为研究对象,选择同时期体检中心体健健康者28名为对照组。对所有受试者行肺功能、血清超氧化物歧化酶(SOD)、血清丙二醛(MDA)含量等进行3个月的监测,并收集长春地区同期各监测点PM 2.5实时监控数据。结果 PM 2.5水平与慢阻肺患者的肺功能呈负相关,其中与第一秒用力呼气容积(FEV1)占预计值百分比和FEV1占用力肺活量(FVC)百分比均呈负相关(r为-0.425和-0.815,P<0.05);与慢阻肺患者SOD、MDA含量呈正相关(r为0.539和0.498,P<0.05)。结论环境PM 2.5污染会影响慢阻肺患者的肺功能,并加重慢阻肺患者的气道炎症和氧化应激反应。 Objective To investigate the association of atmospheric fine particulate matter(PM2.5) pollution with airway inflammation and oxidative stress in patients with chronic obstructive pulmonary disease(COPD) patients. Methods A total of 78 patients diagnosed with COPD in the first hospital of Jilin University from August to September 2013 were chosen and divided into two groups. Patients in one group were from downtown area and those in the other group were from suburban area. Twenty eight normal individuals who come to accept physical examinations at the same period were chosen as controls. Indicators including pulmonary function indexes, serum superoxide dismutase(SOD) levels, serum malondialdehyde(MDA) contents were monitored in all the three groups for 3 months, and the real-time data of PM2.5 levels in Changchun at the same period were collected. Results The level of PM2.5 was negatively correlated with the percentage of the expected value of the forced expiratory volume in one second( FEV1 %) and FEV1 /forced vital capacity(FVC), the correlation coefficient were-0.425 and-0.815(both P〈0.05), while it was positively correlated with the SOD levels and MDA contents, the correlation coefficient were 0.539 and 0.498(both P〈0.05) in patients with COPD. ConclusionPM2.5 pollution affects lung functions in patients with COPD, and aggravates the airway inflammation and oxidative stress as well.
