植入型心律转复除颤器安置术后创新随访模式对患者生存质量及心理状况的影响

Effect of New Follow-up Mode on Quality of Life and Psychological Status among Patients after Implantable Cardioverter Defibrillator Therapy

目的植入型心律转复除颤器(implantable cardioverter defibrillator,ICD)置入术后,比较创新随访模式与传统随访模式对患者生存质量(quality of life,QOL)及心理状况的影响。方法 2014年1月至2015年1月顺序纳入100例四川大学华西医院安置ICD术后患者,随机分为常规随访模式组(n=52)及创新随访模式组(n=48),平均随访(6.5±3.5)月,应用MOS SF-36量表及SCL-90症状自评量表分别对QOL及心理状况进行调查。结果 ICD术后,创新随访模式组QOL评分比常规随访模式组高,而心理状况评分低于常规随访模式组,两组差异有统计学意义(P<0.05)。创新随访模式组中,是否放电、病程长短(以3年为分界点)和学历是QOL及心理状况的影响因素(P<0.05)。结论 ICD术后,创新随访模式可进一步效改善患者的QOL和心理状况。创新随访模式患者中,病程长短、是否放电和学历是心理状况和QOL评分的影响因素,提示创新随访模式更适用于高学历患者。

Objective To analyze the effect of new follow-up mode on quality of life（QOL）and psychological status among patients having undergone implantable cardioverter defibrillator therapy（ICD）.Methods A total of100 patients underwent ICD in our hospital during January 2014 to January 2015 were randomly divided into routine follow-up mode group（n=52）and new follow-up mode group（n=48）.After an average of（6.5±3.5）months′follow-up,patients′overall QOL and psychological were studied using MOS SF-36 scale and symptom checklist-90（SCL-90）.Results The QOL of patients in routine follow-up mode group was significantly lower than that of subjects in new follow-up mode group（P〈0.05）.The psychological status score,which was obtained by the examination of SCL-90,was significantly higher in patients in routine follow-up mode group（P〈0.05）.Discharging,the length of the disease course（cut-off point was 3years）and educational background are the influence factors for the QOL and psychological status among patients in new follow-up group.Conclusion Compare to the routine mode,new follow-up mode can further improve the QOL of patients after ICD.Discharging,the length of the disease course and educational background are the influence factors for the QOL and psychological status among patients in new follow-up group,therefore,new follow-up mode may be more suitable for patients with high educational background.
