剖宫产术后穴位及乳房按摩刺激泌乳的临床应用研究

Stimulating lactation massaging breast and acupuncture points after cesarean section

目的探讨剖宫产术后穴位及乳房按摩刺激泌乳的临床效果。方法选择2013年8月至2015年12月期间我院产科剖宫产术后200例无母乳喂养禁忌症的产妇，按照数字表法随机分为两组，对照组100例在剖宫产术后实施常规护理进行母乳喂养，不用任何催乳方法。观察组100例则在对照组的基础上采用穴位及乳房按摩刺激泌乳方法。结果观察组产妇的泌乳始动时间（20．7±14．5）h，显著地早于对照组的（32．8±16．9）h，两组比较差异有统计学意义（P〈0．05），观察组泌乳量增加较对照组显著，两组比较差异有统计学意义（P〈0．05），观察组新生儿在研究期间的体重变化为（-168．75±68．89）g，对照组新生儿在同一时期的体重变化为（-233．92±72．96）g。观察组新生儿体重下降的幅度明显的小于对照组（t=3．25，P〈0．05）。对照组新生儿中有2人体重下降超过10％，而观察组的新生儿体重下降均在正常的生理性范嗣内。结论剖宫产术后穴位及乳房按摩刺激泌乳具有乳汁分泌时间提前、泌乳量增加的作用，还能缓解产后乳房胀痛。

Objective To explore the clinical effect of stimulating lactation massaging breast and acupuncture points after cesarean section. Methods 200 puerperae without contraindications of maternal breastfeeding after cesarean section were selected from our hospital from August, 2013 to December, 2015 and were randomly divided into a control group and an observation group, 100 for each group. After cesarean section, the control group were routinely cared and maternally breastfed their infants. In addition, the observation group were stimulated lactation massaging breast and acupuncture points. Results The initiating time of maternal lactation was （20.7±14.5） h in the observation group and was （32.8±16.9） h in the control group, with a statistical difference（P 〈 0.05）. The mammary volume increased more in the observation group than in the control group, with a statistical difference（P 〈 0.05）. During the study period, the change of the body weight of neonates was （-168.75±68.89） g in the observation group and was （-233.92±72.96） g in the control group. The neonates of the observation group lost less weight those of the control group （t=3.25, P 〈 0.05）. 2 neonate in the control group lost 10% weight, while all the neonate in the observation group lost weight within normal physiological range. Conclusions Stimulating lactation massaging breast and acupuncture points after cesarean section can stimulate maternal milk secretion, increase mammary volume, and relieve postpartum breast tenderness.
