摘要
目的研究骨桥蛋白在骨化胸椎黄韧带中的表达情况和部位,探讨其在胸椎黄韧带骨化发病中的作用。方法选19例胸椎黄韧带骨化者为实验组,根据病理结果将其分为成熟型与非成熟型骨化;对照组分为2组,正常对照组为9例胸椎骨折患者,退变对照组为8例腰椎间盘突出症患者,均无黄韧带骨化。手术切取标本行病理检查,并行免疫组化测定OPN表达情况及部位。结果在实验组19例患者共46节骨化节段中,成熟型骨化15节,非成熟型骨化31节。骨桥蛋白主要在不成熟型黄韧带骨化中有大量表达,表达部位位于潮线附近肥大的软骨细胞内及临时钙化带表层的基质内;骨桥蛋白在成熟型黄韧带骨化中有少量表达,表达部位主要位于在破骨细胞内;骨桥蛋白在退变黄韧带部分软骨细胞内有少量表达;骨桥蛋白在正常黄韧带基质内有微量表达,对照组与实验组差异有统计学意义(P<0.05)。结论 OPN在骨化的黄韧带不同阶段中均存在表达,并且表达部位和表达量均不同,其可能参与了骨化的整个病理过程。
Objective To study the expression and localization of osteopontin (OPN) was clarified in this research so as to ex- plore its relationship with the etiology of OTLF. Methods This investigation involved 19 patients who involved 46 spinal segments with symptoms of myelopathy or radiculopathy due to the compression of TOLF as experimental group. The posterior floating and en bloc resec- tion technique was performed for all patients. Meanwhile, control specimens of ligamentum flavum were obtained from 17 patients. In these cases,8 patients with lumber stenosis and 9 patients with thoracic fracture. OPN were stained by the immunohistochemical (IHC) stai- ning techniques using specific antibodies. Results The samples of OLF in the experimental group were divided into mature ossification and immature ossification type. In all 46 samples,there were 15 mature ossification samples and 31 immature ossification samples. The mainly expression instance of osteopontin in immature ossification of ligamentum flavum:it expressed in the surface layer of the temporary calcification zone. There was a small quantity of OPN expression in the matrix of the ligamentum flavum,large numbers of OPN expression in the hypertrophic chondrocyte near to tide lines. Simultaneity, the OPN expression was detected and formed calcification regions. In mat- uratic ossification of ligamentum flavum, the OPN expression was detected in the cell plasm of osteoclast attaching cement hne which was located in Hovers duct. The marked discrepancy existed between two groups. In control patients, OPN was positively stained in chondro- cyte partly, but a small quantity of OPN expression in the matrix. There were much less OPN expression in non - degeneration groups. The expression osteopontin in ossification of ligamentum flavum was deeper than the control group. The marked discrepancy existed be- tween two groups ( P 〈 0.05 ). Conclusion The OPN is positively stained in the OLF and may be effect all the ossification process.
