妊娠期糖尿病的社区个体化综合干预的效果分析

Effect analysis of community individualized integrated intervention for gestational diabetes mellitus

目的探讨个体化综合管理在妊娠期糖尿病患者中的临床管理效果。方法 80例妊娠期糖尿病患者,根据不同管理方案为对照组和实验组,各40例。对照组采用常规方法管理,实验组实施社区个体化综合干预管理,比较两组管理效果。结果两组患者管理前空腹血糖和餐后2 h血糖差异不显著(P>0.05);实验组管理后空腹、餐后2 h血糖显著低于对照组(P<0.05);实验组2例早产儿,1例胎儿窘迫,2例新生儿低血糖,显著低于对照组(1例巨大儿,5例早产儿,3例胎儿窘迫,2例新生儿窒息,5例新生儿低血糖)(P<0.05);实验组并发症发生率为7.5%显著低于对照组17.5%,差异具有统计学意义(P<0.05)。结论妊娠期糖尿病患者治疗过程中实施社区个体化综合管理干预效果理想,能够改善患者血糖、改善妊娠结局,值得推广使用。

Objective To investigate clinical management effect of individualized integrated management in patients with gestational diabetes mellitus.Methods A total of 80 patients with gestational diabetes mellitus were divided by different management methods into control group and experimental group, with 40 cases in each group. The control group received conventional method for management, and the experimental group received community individualized integrated management. Management effects of the two groups were compared.Results There was no statistically significant difference of fasting blood-glucose and 2 h postprandial blood glucose between the two groups before management （P＆gt;0.05）. After management, the experimental group had much lower fasting blood-glucose and 2 h postprandial blood glucose than the control group （P〈0.05）. There were 2 premature infant cases, 1 fetal distress case, and 2 neonatal hypoglycemia cases in the experimental group, and they were much less than the control group （1 macrosomia case, 5 premature infant cases, 3 fetal distress cases, 2 neonatal asphyxia cases, and 5 neonatal hypoglycemia cases） （P〈0.05）. The experimental group had obviously lower incidence of complications as 7.5% than 17.5% of the control group, their difference had statistical significance （P〈0.05）. Conclusion Implement of community individualized integrated management provides ideal effect for treating gestational diabetes mellitus, and it can improve blood glucose and pregnancy outcome in patients. This method is worthy of promotion and application.
