摘要
目的:评价北京市基本药物合理使用情况,并提出相关可行性建议。方法:根据北京市2013年糖尿病领域社保抽样处方数据,并结合4家基层医疗机构处方数据、实地调研和借鉴选择性用药指标,对北京市基本药物合理使用情况进行评价。结果与结论:按医疗机构级别,0级医疗机构单张处方的药品数最少,1级医疗机构最多;基层医疗机构抗生素使用较多,三级医疗机构激素使用较多;医院级别越高,基本药物的使用比例越低,单张处方的费用越高;按城乡划分,与市区相比,乡村抗生素使用较多,激素使用较少,基本药物使用比例较低,处方费用较低。北京市基本药物的使用合理性还有很大提升空间,建议加大基本药物配套政策的执行力度,严格控制药品的不合理使用。
Objective： To analyse the rational use of essential medicine in Beijing, and to put forward relevant feasible suggestions.Methods：Data were collected from a sample of a social security database on diabetes in 2013 and prescription data from a ifeld research on 4 primary healthcare facilities. The evaluation on rational drug use was based on Selected Drug Use Indicators （SDUIs）.Results and Conclusion：Sorted by hospital levels, level-0 hospitals used least drugs per prescription, while level-1 hospitals used most drugs; primary hospitals used more antibacterial; level-3 hospitals used more hormone; as the hospital level rose, the proportion of use of essential medicine declined and drug expenditure per prescription increased. Compared with urban hospitals, rural hospitals used more antibacterial, less hormone, less essential medicine, and had lower drug expenditure per prescription. Rational use of essential medicine in Beijing still needs to be improved. It is suggested that Beijing strengthen supporting policies of essential medicine and control the irrational drug use.
