CT检查对良恶性骨肿瘤或肿瘤样病变的鉴别诊断价值 被引量:28

Differential diagnosis value of X-ray and computed tomography for benign-malignant bone tumor or tumor-like lesion

摘要 目的探讨CT检查对良恶性骨肿瘤或肿瘤样病变的鉴别诊断价值。方法回顾性分析2012年1月至2014年1月间收治的110例骨肿瘤或肿瘤样病变患者的X线和CT检查资料,与病理结果进行对照,分析二者对良、恶性骨肿瘤或肿瘤样病变的诊断情况。结果病理结果显示,110例患者中,良性骨肿瘤56例,恶性骨肿瘤41例,肿瘤样病变13例,X线诊断符合率为70.0%(77/110),CT诊断符合率为82.7%(91/110),两者联合诊断符合率为97.3%(107/110),CT诊断符合率显著高于X线,两者联合诊断符合率显著高于单独检查,差异均有统计学意义(P<0.05)。X线的检查时间为(27.6±3.4)min,CT的检查时间为(28.3±3.1)min,差异无统计学意义(P>0.05);X线的检查费用为(72.9±12.0)元,CT的检查费用为(551.5±46.4)元,差异有统计学意义(P<0.05)。结论 X线和CT检查对良、恶性骨肿瘤或肿瘤样病变均有明显的影像学特征,具有重要的鉴别诊断价值,X线检查简便、经济,CT检查诊断符合率更高,临床医师应该根据患者的具体情况选择合适的检查方案。 Objective To explore the differential diagnosis value of X-ray and computed tomography( CT) for benign-malignant bone tumor or tumor-like lesion. Methods From January 2012 to January 2014,the X-ray and CT data of 110 patients with bone tumor or tumor-like lesion were retrospectively analyzed,they were compared with pathological results,and the diagnosis data of benign and malignant bone tumors or tumor-like lesions were analyzed. Results Totally 110 patients' pathological examination showed that 56 cases were benign bone tumors,41 cases were malignant bone tumors,and 13 cases were tumor-like lesions. The diagnose accordance rate of X-ray was 70. 0%( 77 /110); The diagnose accordance rate of CT was 82. 7%( 91 /110); Combined diagnostic accordance rate was 97. 3%( 107 /110). The diagnose accordance rate of CT was significantly higher than X-ray,and combined diagnostic accordance rate was significantly higher than single examination,with significant difference( P 0. 05). The inspection time of X-ray was( 27. 6 ± 3. 4) min,the examination time of CT was( 28. 3 ± 3. 1) min,and the difference was not statistically significant between two groups( P 0. 05); The inspection fees of X-ray was( 72. 9 ±12. 0) yuan,inspection fees of CT was( 551. 5 ±46. 4) yuan,and the difference was statistically significant between two groups( P 0. 05). Conclusions X-ray and CT examinations of benign and malignant bone tumors or tumor-like lesions have significantly imaging features,and have an important differential diagnosis. X-ray examination is simple,more economical,while CT examination has higher diagnosis rate. Clinicians should selecte the appropriate inspection program based on the the patient's specific circumstances.
