腹腔镜与开腹行进展期胃癌 D2根治术的临床对比报告

Clinical comparison of laparoscopic and laparotomy D2 radical resection for advanced gastric cancer

目的：探讨腹腔镜与开腹行进展期胃癌D2根治术的临床效果。方法选取2013年9月至2014年9月收治的120例胃癌患者作为研究对象，按照随机分组法将其分为对照组和观察组，每组60例，比较腹腔镜与开腹手术治疗进展期胃癌患者的临床疗效。结果与对照组比较，观察组手术时间相对较长，差异有统计学意义（P＜0.05）；观察组住院时间、术中出血量、术后胃肠道功能恢复时间均明显优于对照组，差异有统计学意义（ P＜0.05）；两组淋巴结清除数目比较差异未见统计学意义（ P＞0.05）。结论与开腹手术比较，腹腔镜直视下行进展期胃癌D2手术虽具有同样的手术治疗效果，但腹腔镜手术具有住院时间短、出血量少、胃肠道功能恢复快以及术后疼痛少等优点。

Objective To investigate the clinical effects of laparoscopic and laparotomy D2 radi-cal resection on advanced gastric cancer.Methods One hundred and twenty cases of gastric cancer in our hospital from September 2013 to September 2014 were treated during the study according to the ran-domization method, and they were divided into the control group and the observation group with 60 cases in each group.The clinical efficacy of patients with advanced gastric cancer in the two groups were com-pared.Results Compared with the control group, the operation time was longer in the observation group, the difference was significant （P〈0.05）;the hospitalization time, blood loss, postoperative gastrointesti-nal function recovery time in the observation group was significantly superior to those in the control group, the differences were significant （ P〈0.05 ）; there was no significant difference in the lymphadenectomy number between the two groups （ P〉0.05） .Conclusions Compared with open surgery, laparoscopic D2 radical resection for advanced gastric cancer has the same effect, but laparoscopic surgery has shorter hos-pital stay, less blood loss, faster recovery of gastrointestinal function and less postoperative pain.
